ENS 5416415 July 2019 04:00:00Agreement StateAgreement State Report - Damaged Troxler Gauge Due to Truck FireThe following report was received from the Ohio Bureau of Environmental Health and Radiation Protection via email: The licensee (Professional Service Industries, Inc.) experienced a vehicle fire today (7/15/19) involving a Troxler 3430P gauge, S/N 70185, containing a double-encapsulated sealed source of Cs-137 (8 mCi (0.30 GBq)) plus a double-encapsulated sealed source of Am-241 (40 mCi (1.48 GBq)). The vehicle driver pulled over on the shoulder of the interstate when he noticed smoke (coming) from the bed of the truck. The licensee believes that the cause of the fire may have been as a result of vandalism that ruptured the fuel tank. The licensee reported that such activities have occurred in the past. No one was injured. After the fire was extinguished, the device was surveyed using a calibrated NDS Model ND-500A survey meter. The measured radiation levels of the device were 20 mR/hr at the surface, and 0.9 mR/hr at 1 meter, without the transport case. The device has been leak tested pending results and Troxler has been contacted for disposal. Item Number: OH190011
ENS 461505 August 2010 11:30:00Agreement StateAgreement State Report - Stolen Troxler Moisture Density GaugeThe following information was received from the State of Florida via fax: A soil moisture density gauge (Troxler) was discovered stolen at approximately 0730 (EDT) on 5 Aug 2010, this office notified at 1015 (EDT). Gauge stored in a locked conex box, case chained to the conex box, case was locked and gauge itself was locked; owner still has control of gauge's key. Conex box lock found broken and chain lock broken also. Licensee will offer a reward. Orlando Inspection Office will investigate. This office will take no further action on this incident. The incident location was a jobsite on Dunn Avenue 3/4 mile west of Williamson Blvd. in Daytona Beach. Daytona Beach PD case number 2010016305. The stolen gauge is a Troxler Model 3430, S/N 26007, containing two (2) sources; 40 mCi Am-241/Be and 8 mCi Cs-137. Florida Incident Number: FL10-088 THIS MATERIAL EVENT CONTAINS A "LESS THAN CAT 3" LEVEL OF RADIOACTIVE MATERIAL Sources that are "Less than IAEA Category 3 sources," are either sources that are very unlikely to cause permanent injury to individuals or contain a very small amount of radioactive material that would not cause any permanent injury. Some of these sources, such as moisture density gauges or thickness gauges that are Category 4, the amount of unshielded radioactive material, if not safely managed or securely protected, could possibly - although it is unlikely - temporarily injure someone who handled it or were otherwise in contact with it, or who were close to it for a period of many weeks.
