ENS 5692211 January 2024 06:00:00Agreement StateSource Stuck in Unshielded PositionThe following information was provided by the Texas Department of State Health Services (the Department) via phone and email: On January 11, 2024, the licensee's service company reported to the Department that when the licensee's radiation safety officer was verifying a lock-out on an Ohmart SHLM-BR-2 fixed gauge, containing a 1,000 millicurie cesium-137 source, he found that the source had separated from the insertion rod and therefore could not be returned to the fully shielded position. The gauge operates by rod insertion of the source into a housing that is inside the reactor unit. The licensee has contacted the manufacturer to get the gauge repaired. There have been no exposures as a result of this event, and none are expected due to the location of the gauge. The intended work on the unit has been postponed until the gauge is repaired and the source can be returned to the fully shielded position. An investigation into this event is ongoing. ENS 5375828 November 2018 06:00:00Agreement StateAgreement State - Fixed Gauge Stuck ShutterThe following information was received from the State of Texas via email: On November 28, 2018, the Agency (Texas Department of State Health Services) was notified by the licensee that while performing a shutter test on an Ohmart-Vega Model SH-F1 gauge containing a 20 milliCurie (original activity) Cs-137 source, the shutter would not close. Open is the normal operating position for the gauge. The gauge is in a location that does not present an exposure risk to any individual. The licensee has contacted a service provider to repair the gauge. Additional information will be provided as it is received in accordance with SA-300. Texas Incident #: I-9640.
ENS 5237316 November 2016 20:00:00Agreement StateAgreement State Report - Stuck Shutter on Fixed Nuclear GaugeThe following was excerpted from information received from the Texas Department of State Health Services by email: On November 17, 2016, the licensee notified the Agency (Texas Department of State Health Services) that on November 16, 2016 at approximately 1400 CST, during its semi-annual fixed nuclear gauge inspection (at Chocolate Bayou Works), it had discovered that the shutter mechanism on an Ohmart Model SHLM-BR-2, which contains a 1,000 millicurie cesium-137 source (SN: 2807CG), had become inoperable. The licensee's radiation safety officer reported the source could be moved up/down inside the source tube, but it could not be retracted into the housing. He suspects there is an issue with the positioning of the pin that holds the 2-piece source rod together. Repairs are scheduled to be completed during an operations shut-down in January/February 2017. The source is in its normal operating position and there is no risk of exposure to any individual due to its location. Texas Incident #: I-9441
ENS 4684311 May 2011 05:00:00Agreement StateAgreement State Report - Operating Mechanism Failed on Gauge

The following information was obtained from the State of Texas via email: On May 12, 2011, the Agency (state) was notified that the operating mechanism on an Ohmart Model SHLM-BR-2 nuclear gauge containing 1,000 milliCuries of Cesium (Cs) 137 had failed. The operating mechanism consists of two rods. The source is mounted on one rod and the other rod is used to move the source into and out of the source holder. The pin used to connect the two rods together broke so the source can not be manipulated. The source is currently located inside the vessel in its normal operating position. There is no increased risk of radiation exposure to any individual. The licensee is making arrangements to have the gauge repaired. The cause for the failure is still being investigated. Additional information will be provided as it is received in accordance with SA-300.
