ENS 560988 September 2022 05:00:00Agreement StateEquipment Failed to Function as DesignedThe following was submitted by the TX Department of State Health Services (the Agency): On September 8, 2022, the Agency was notified by the licensee that on this day, the shutter on a Vega SH-F2 gauge containing a 500 milliCuries (original activity) cesium - 137 source failed to close during routine testing. Open is the normal operating position of this gauge. The licensee will contact the manufacture to repair the shutter. No individual received additional exposure due to this event. ENS 527302 May 2017 19:00:00Agreement StateAgreement State Report - Level Density Gauge Shutter Stuck OpenThe following information was obtained from the state of Louisiana via email: On 05/03/2017, the Assistant Radiation Safety Officer (RSO) for ExxonMobil Chemical received notice that a level density gauge was having difficulty with the operation of the shutters on a gauge installed on a process. During follow-up checks, the one level gauge was found with shutters that were stuck in the open position and the manual operation handle had broken. The report was received by the RSO at approximately 1400 (CDT) on May 02, 2017. The gauge was an Ohmart Corporation Gauge, devices involved, Model Number SHF-2-45. The gauge/device S/N unknown is loaded with approximately 200 mCi of Cs-137 and the S/N 5828GK. The source and source holder usually have one S/N for the whole device. BBP Sales/Service Company was contacted to fix the problem by repairing the gauge or replacing the device. The gauge is installed on processes and does not pose a health and safety threat to the general public or the employees. The source survey revealed the readings were less than 2 mR/hr and proper safety precautions will be taken when personnel enters the area. This is considered an equipment failure with the manual shutter handle device breakage. LA Event Report ID No.: LA-170007
ENS 4898722 April 2013 05:00:00Agreement StateAgreement State Report - Shutters Found That Were Difficult to Operate on Two Process GaugesThe following was received from the State of Louisiana via facsimile: On 4/22/2013, the Radiation Safety Officer (RSO) for ExxonMobile Chemical was performing his annual inventory and operational checks on the level gauges at the Baton Rouge facility. During the checks, two level gauges were found with shutters that were difficult to operate. The gauges detected were Ronan Engineering Gauges, devices involved, Model Number SAF-F37. One gauge is S/N 0781GK loaded with 100 mCi of Cs-137 and S/N 4585GH loaded with 25 mCi of Cs-137. Ronan Engineering has been contacted to fix the problem or repair the gauge. These gauges are installed on process and pose no safety threat to the general public or the employees. This is not considered a safety equipment failure, just being installed on a process in a very harsh corrosive environment. This is a two shutter malfunction on level gauges installed on processes. These devices do not pose a radiation exposure threat to the general public. They will remain in operation on the process until and after repair. The department considers this event closed.
