ENS 5186312 April 2016 17:35:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseCourtesy Offsite Notification Made to State and County Officials

On April 12, at 1235 CDT, Duane Arnold Energy Center contacted officials with the State of Iowa (Bureau of Radiological Health and Department of Natural Resources) and Linn County Public Health Department in accordance with the nuclear industry voluntary reporting criteria contained in NEI 07-07 'Industry Ground Water Protection Initiative'. The site contacted the agencies as courtesy to notify them about the identification of low levels of tritium found within the site's protected area from a potential new source. Samples were taken, and no regulatory limits were exceeded. The site team is working with industry experts on pinpointing the cause and installing an extraction well to remediate the situation. This report is being made in accordance with 10 CFR 50.72(b)(2)(xi), as a result of notification to offsite agencies.

The Licensee has notified the NRC Resident Inspectors. There is no risk to plant employees, the public or drinking water.

ENS 495218 November 2013 19:40:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Onsite Death of an EmployeeAt approximately 1104 CST on November 08, 2013, in the Owner Controlled Area, a station employee fell ill from an apparent personal medical condition. The employee was transported off site and passed away. The death was not related to activities impacting or related to the safety of the public or other on site personnel. Specifically, the individual collapsed and was promptly examined by Duane Arnold Energy Center Emergency Medical Technician(s). An ambulance evacuation was then ordered. The death was not work-related nor the result of an accident. The individual was outside of the Radiological Controlled Area and no radioactive material or contamination was involved. Duane Arnold Energy Center has not observed any heightened public or media concern as a result of the death. Since the death is apparently unrelated to Duane Arnold Energy Center industrial or radiological health and safety, no news release is planned. The individual's name has not yet been released, pending notification of next-of-kin. The Iowa Division of Occupational Safety and Health was notified of the death at 1340 CST. The licensee notified the NRC Resident Inspector.
ENS 4840312 October 2012 20:35:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Leak from Condensate Storage Tank Containment Pit SumpOn October 12, at 1535 CDT, Duane Arnold Energy Center made a voluntary report to the State of Iowa (Bureau of Radiological Health and Department of Natural Resources) and Linn County Public Health Department in accordance with the nuclear industry voluntary reporting criteria contained in NEI 07-07, 'Industry Ground Water Protection Initiative.' The subject of the report was a leak from the Condensate Storage Tanks' containment pit sump. The release occurred in an isolated area inside the plant property and has been terminated. Samples were taken and no regulatory limits were exceeded. During and following the leak, there was no impact to site worker or public health and safety. This report in being made in accordance with 10 CFR 50.72(b)(2)(xi), as a result of notification to offsite agencies. The Licensee has notified the NRC Resident Inspector. Total amount leaked was estimated to be less than 100 gallons.
ENS 4661917 February 2011 20:39:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Noitification Due to an Alarming Emergency SirenAt 1439 (CST), on 2/17/2011, the NextEra Energy Duane Arnold control room was informed that an emergency siren in the Cedar Rapids area was alarming. This information came via the Cedar Rapids Police Department and Linn County Emergency Management Agency who stated that they had been informed that a siren was alarming. No emergency condition exists at NextEra Energy Duane Arnold and efforts are currently underway to determine if in fact an emergency siren is alarming. The licensee notified the NRC Resident Inspector and the local press.
ENS 4610417 July 2010 02:34:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Emergency Siren Inadvertently AlarmingAt 2134 (CDT) on 7/16/2010 the NextEra Energy Duane Arnold control room was informed that an emergency siren in the Cedar Rapids area was alarming with an emergency message. This information came via a local news station stating that they had been receiving several phone calls regarding the siren message. No emergency condition existed at NextEra Energy Duane Arnold and the siren appears to have malfunctioned and incidentally relayed the emergency message. NextEra Energy Duane Arnold has informed local media and the Linn County Emergency Director of the malfunctioning siren, and that no emergency condition exists at NextEra Energy Duane Arnold. The single siren (of the 144 total in the local siren system) was deactivated. The licensee notified the NRC Resident Inspector.
ENS 436259 September 2007 13:30:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleasePress Release Due to Linn County Inadvertent Emergency Siren ActivationA press release is being made by Linn County stating that an unknown number of Emergency Notification sirens inadvertently activated in the Cedar Rapids area. The cause for the sirens being inadvertently activated is being investigated by utility employees. These sirens are used for warning the public during severe weather, an emergency at the Duane Arnold Energy Center, or other type of community emergencies. The sirens were not activated by any of the normal control locations in Linn and Benton counties. The sirens were activated for an unknown amount of time and have been secured. Licensee has been in communication with local emergency planning organizations. The licensee notified the NRC Resident Inspector.
ENS 410122 September 2004 19:15:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Regarding Release of OxidantOn September 2, 2004 at 14:15 (CDT) the Duane Arnold Energy Center made notification to Mr. Mike Wade of the Iowa Department of Natural Resources regarding a release of free available oxidant (chlorine) to the Cedar River. Although, the initial indications show that this release did not exceed our NPDES discharge permit value, as stated by Mr. Wade of the IDNR, the notification to an outside government agency meets the criteria of a 4-hour notification to the NRC per 50.72(b)(2).(xi). The licensee notified the NRC resident inspector.
