ENS 5677826 September 2023 05:00:00Agreement StateFixed Gauge Stuck ShutterThe following information was provided by the Texas Department of State Health Services (the Agency) via email: On October 5, 2023, Covestro LLC (the licensee) reported to the Agency that on September 26, 2023, during routine 6-month checks, the shutter on one of its VEGA SHLG-1 fixed nuclear gauges, containing 1.5 curies cesium-137, was found stuck in the open position. Open is the normal operating position for this gauge. There were no (personnel) exposures and none are anticipated as the gauge is mounted on the side of a vessel and secured from access. The licensee has contacted the manufacturer and is trying to make arrangements for repair or replacement. An investigation into this event is ongoing. More information will be provided as it is obtained in accordance with SA-300. Texas Incident Number: 10056
ENS 5643728 March 2023 05:00:00Agreement StateStuck ShutterThe following report was received from the Texas Department of State Health Services (the Agency) via email: On March 29, 2023, the licensee reported to the Agency that on March 28, 2023, one of its technicians was performing routine shutter checks, and the shutter on one of their Vega SHF-2 gauges, containing a 50 millicurie cesium-137 source, was stuck in the open position. Open is the normal operating position for the gauge. The gauge is mounted 10-12 feet above the ground on the side of a tank with no direct access so there is no risk of exposure to individuals The licensee will contact a service company to make repairs. EN Revision Text: AGREEMENT STATE REPORT - FIXED GAUGE STUCK SHUTTER The following report was received from the state of Texas via e-mail: On March 21, 2019, the licensee notified the Agency (Texas Department of State Health Services) that during routine shutter checks on March 20, 2019, it had discovered that the shutter on one of the Ronan Model SA-1 gauges (SN: M-3824) was stuck in the open position. Open is the normal operating position for this gauge. The gauge contains a cesium-137 source with a calculated current activity of 10 milliCuries. Due to the location of the gauge, there is no increased risk of exposure. The licensee plans to replace the gauge. Texas Incident #9667

The following update was received from the Texas Department of State Health Services via email: Licensee contacted the Agency on October 9, 2019, to report that the vendor was successfully able to close the stuck shutter and the old source is being packaged and prepared for disposal. The gauge was replaced with a Vega SHLD-1. File is closed. Notified the R4DO (Kellar) and NMSS Events (via email).

ENS 5173111 February 2016 06:00:00Agreement StateAgreement State Report - Fixed Gauge Shutter Found in Stuck Open PositionThe following information was provided by the State of Texas via email: On February 12, 2016, the Agency (Texas Department of State Health Services) was notified by the licensee's radiation safety officer (RSO) that on February 11, 2016, while performing a routine inspection of a Berthold model LB 300 l nuclear gauge, they found the source shutter was stuck in the open position. Open is the normal position for the gauge shutter. The gauge contains a one millicurie (original activity, current activity 0.07 mCi.) Cobalt-60 source. The RSO stated the dose rates at and around the gauge are normal. He (the RSO) stated no individuals, including members of the general public, will be exposed to any additional radiation due to the failure. The RSO stated the manufacturer has been contacted and will be at the licensee's facility the week of February 15, 2016 to fix or replace the gauge. Additional information will be provided as it is received in accordance with SA-300. Texas Incident #: I-9378
