1.0 Emergency Core Cooling (ECC) Systems Outage From January 1, 1988 to-December 31, 1988 there were 41 limiting. conditions for operations (LCO's) for the ECC Systems.
Fourteen (14) of the 41 LCO's were actual requirements per Technical Specifications.
The remaining 27 were for tracking.
At Shoreham, an LCO is categorized as a
" tracking LCO" if the technical specification LCO occurred while the reactor was in an operational mode other than that for'which the LCO is applicable.
Tracking LCO's are important because they provide assurance that operability will be restored prior to entering the applicable operational mode.
Of the 14 actual LCO's the breakdown is as follows:
3 are for Ell, Low Pressure Coolant Injection (LPCI) 6 are for E21, Core Spray (CS) 5 are for Ell and E21 combined, LPCI and CS These 14 actual LCO's are detailed in Table 1-1 indicating for each component by system, the outage dates, duration, and the reason for the outage.
Thirteen (13) of the 14 actual LCO's placed the system inoperable due to surveillance testing.
One LCO declared LPCI loop B inoperable for 2.5 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br /> to retorque an end cap on a snubber per Engineering and Design Coordination Report (E&DCR) L-0422.
Two of the surveillance related LCO's found the instrument out of calibration.
They are:
LCO A88-002:
pressure differential switch, 1E11*410PDS-001A for LPCI loop A minimum flow was below the setpoint but within the allowable limit.
The switch was recalibrates.
LCO A88-128:
flow indicating switch, 1E21*410FIS - 002B for Core Spray loop B minimum flow was above the setpoint but within the allowable limit.
The switch broke while recalibrating.
The torque spring detached.
The switch was repaired and recalibrates.

Technical Specification Sections.6.9.1.4 and 6.9.1.5.-
The direct reading dosimeter (DRD) recordings.on the Radiation Work Permits (RWP's) were insignificant and.-
reported as zero.- The Totals and Grand Totals are measured by-the thermoluminescent dosimeters'(TLD's)..
For'this reporting period,ol/1/88'through' 12/31/88,Lno individual was exposed.to more than 100 mrems/yr.
Therefore,.actableLindicating personnel exposures greater than 100 mrems/yr is not included in this report..
