- Chagnon, Larry Pinnacle Investigations Pursuant to Title 10 CFR 9.23, Freedom of Information Act (FOIA), I'd like to request the complete OI investigation report, including all supporting documentation, regarding NRC Office of Investigation Report No. 4-2024-007. This report references an OI investigation conducted between December 2023 and December 2024. The purpose of the investigation was to determine if a former employee of Pinnacle Investigations knowingly provided services that would have caused, if not detected, a licensee to be in violation of an NRC regulation.

The purpose is to heighten awareness of recently observed inconsistencies between the language in licensee programs during implementation of ASME Code Case N-752 associated with risk-informed categorization and treatment for repair or replacement activities in safety-related Class 2 and 3 structures, systems, and components (SSCs) and regulatory requirements regarding compliance with 10 CFR Part 50, Appendix B quality assurance criteria.
Holtec Decommissioning International, LLC Submitted a License Amendment Request to Allow for Sleeving of Steam Generator Tubing as a Repair Method at Palisades Nuclear Plant On February 11, 2025 (added to ADAMS on February 13th), Holtec Decommissioning International, LLC submitted a license amendment request to allow for sleeving of steam generator tubing as a repair method at Palisades Nuclear Plant. The licensee has asked for the staff to complete the review by August 15, 2025. The staff has begun its acceptance review of the application.
