OPERATING LICENSE NO. NPF-12 SPECIAL REPORT (SPR) 2016-001 South Carolina Electric & Gas Company (SCE&G) is submitting this Special Report pursuant to the requirements of Technical Specifications 6.9.2 as required by Action 30 of Technical Specifications 3.3.3.1.b, Table 3.3-6, RADIATION MONITORING INSTRUMENTATION, Item 2.d.i ii.

On April 26, 2016, at 0100 hours0.00116 days <br />0.0278 hours <br />1.653439e-4 weeks <br />3.805e-5 months <br />, the Reactor Building Purge Exhaust Effluent High Range Radiation Monitor (RM-A 14) was declared inoperable due to erratic spiking indications. Upon declaring RM-A14 inoperable, VCSNS implemented a preplanned alternate method of monitoring in accordance with the requirements of Action 30 of the Technical Specifications.

After troubleshooting, failure of RM-A 14 was determined to be due to a failed detector/preamp assembly. The detector/preamp assembly was repaired, satisfactory retested, and returned to operable status on 05/03/16 at 1805 hours0.0209 days <br />0.501 hours <br />0.00298 weeks <br />6.868025e-4 months <br />.

RM-A 14 provides radiation monitoring for the Reactor Building Purge Exhaust System as described within the FSAR section 9.4.8.2.2 Reactor Building Purge Supply and Exhaust System, and FSAR Figure 9.4-28.
