dated June 14, 2021 On April 27, 2021 a blind quality assurance specimen was submitted to a Health and Human Services (HHS) certified laboratory. The test results returned from the HHS lab on May 2, 2021 were inconsistent with the expected results. The specimen correctly identified positive results for two drugs, but incorrectly identified a negative result for one other drug. This was not considered to be a false positive test nor a false negative test. The NRC Resident Inspector was briefed on the potential blind sample error.
The investigation results were returned to River Bend Station on June 14, 2021. The investigation revealed that laboratory personnel error was the root cause of the inconsistent test results. Investigation results are documented in the enclosure attached to this letter.
