3 88012 EI SWGR Personnel error during DC electrical lineup changes resulted in a loss of feedwater, leading to a reactor scram on low reactor water level. Plant systems responded as designed, water level was restored, and the plant was stabilized in Hot Shutdown.

3 88015 SE ISV A procedural deficiency during reactor startup resulted JI PCV in steam bypass valve operation upon opening MSIV's, leading to a reactor scram on low reactor water level.

Applicable operating instructions have been revised.

SUMMARY

Perry-1 experienced I forced outage in April due to personnel error. During_a subsecuent reactor startuo a reactor scram occurred due to a procedural deficiency. At month's end, Perry-1 was in Hot Shutdown, with preparations underway for reactor startup on May 1.
