The information is as initially received without verification or evaluation, and is basically all that is known by the Region III staff on this date.

ALERT DECLARED DUE TO SAFETY RELIEF VALVE FAILING TO CLOSE DESCRIPTION:
At 1:37 p.m. on Wednesday, April 16, Unit 2 of the Quad Cities Nuclear Power Station manually shut down the reactor because a power operated relief valve (PORV) in the containment building unexpectedly opened and failed to close.
The licensee made attempts to close the PORV, but it remained open. The PORV discharges into the torus (suppression pool). When temperature in the torus reached 95F, the reactor was manually scrammed. At 1:59 p.m. the licensee declared an ALERT, according to its emergency procedures. The PORV remains open and suppression pool cooling is operating. Plant personnel and safety equipment responded properly. Unit 1 continues to operate at full power.
The licensee is investigating the cause of the malfunction and making repair plans.
NRC Region III manned its Incident Response Center and entered Monitoring Phase at 2: 35 p.m.
Appropriate notifications have been made to state, federal and local officials.
The NRC was Operations Center was notified at 2:47 p.m.
The information in this preliminary notification has been reviewed with utility management.
