MEDICAL EVENT INVOLVING DOSE TO FETUS On October 20, 1998, Region'II received telephonic notification from the licensee of a medical event that occured at the licensee's facility in Parkersburg, West Virginia.

The licensee reported that, on September 1, 1998,-a female patient was administered 9.2 millicuries of sodium iodide I-131 for the treatment of thyroiditis in accordance with the written directive prepared by an authorized user.

On October 5, 1998, the patient reported to the licensee that she was five months pregnant and, l

'therefore, four months pregnant at the time of the administration.

The licensee contacted a pediatric endocrinologist in Oak Ridge, Tennessee, for assistance in calculating the dose to the thyroid of the fetus.

Using the information provided by the licensee, the dose to the thyroid of the fetus was calculated to be 8,800 rads.

i The licensee reported that their investigation of this event revealed that the licensee followed all established procedures for the therapeutic administration of iodine-131, including ensuring that the patient was i

interviewed regarding pregnancy status, having the patient certify that i

she was not pregnant, and having the patient sign an informed consent l

l form prior to the procedure.

The licensee notified the referring physician of the event and will contact the Radiation Internal Dose Information Center (RIDIC) in Oak Ridge, Tennessee, for further assistance in evaluating the whole body dose and the thyroid dose to the fetus.
