INADVERTENT INCINERATION OF SMALL CALIBRATION SOURCES-On.Moy 14, 1997 the licensee's Alternate Radiation Safety Officer and

-Administrator notified the NRC of the discovered loss of six sealed

-cources that were formerly used to calibrate their dose calibrator. The ccurces.were being stored with biomedical waste and, on April 15, 1997, waro inadvertently transferred to Celsius, Inc., who subsequently incinerated the sources as biomedical waste at the company's Caguas, PR, fccility.

The sources ~ consisted of 77.5 and 86.1 microcuries of cocium-137, 28 and 17.5 microcuries of cobalt-60, and 0.47 and 0.03

.microcuries,of cobalt-57 (not regulated by NRC)..The licensee reported 4

that a series of errors led to the event. The licensee is investigating tho incident and intends to develop an action plan to prevent recurrence.

t The licensee also plans to survey the incinerator for radioactive centamination. Additionally, the licensee indicated that a written report i.cn'this incident will be' submitted as required by 10 CFR 20.2201.

Tho Commonwealth of Puerto Rico has been notified. The Region will centinue to monitor the licensee's actions and will perform a followup insp;ction.
