The information is as initially received without verification or evaluation, and is basically all that is known by the Region I staff on this date.

UPDATE - UNUSUAL EVENT DECLARED DUE TO EXPLOSION IN 31 MAIN POWER TRANSFORMER AT INDIAN POINT UNIT 3 This preliminary notification supplements information in PNO-I-07-004, which documented the declaration of an Unusual Event at Indian Point Unit 3 on April 6, 2007, following a transformer failure and fire. The Unusual Event was entered at 11:43 a.m., based on eyewitness reports of an explosion preceding a fire on the 31 main power transformer, and was subsequently exited at 12:47 p.m.

On April 7, 2007, while the Unit was shutdown, Entergy commenced activities to inspect both the 31 and 32 main power transformers. The 32 main power transformer sustained minor damage due to the explosion and fire on the 31 main power transformer. The apparent cause of the explosion and fire was a failure of the B-phase, high-voltage insulating bushing.

Entergy replaced the 31 main power transformer with a spare and performed repairs on the 32 main power transformer. Following maintenance and testing activities, Entergy plans to restore Indian Point 3 to full power operation.

The NRC staff continues to closely follow Entergys causal evaluations and corrective actions.

These activities will be conducted through the NRCs normal independent baseline inspections.

The information presented herein has been discussed with the licensee, and is current as of 7:00 a.m., April 25, 2007. This preliminary notification is issued for information only and no further notification by the staff is anticipated.
