Kuczynski Telephone (717)S42-2181 Challenges to Main Steam Safety Relief Valves None Changes to the Offsite Dose Calculation Manual None Major Changes to Radioactive Waste Treatment Systems None Diesel Generator Unit Failures During the performance of a monthly operaoility test, Diesel Generator 'D' Lexperienced a' crank case explosion that was apparently caused by an over heating condition in the 2L cylinder.
Inspection of the 2L piston revealed heat discoloration, excessive wear grooving and metal displacement.
The cylinder liner had displaced metal clong the entire length of the piston travel; the rings contained numerous metal filings. Investigation into the cause of this failure include the examination of the 2L piston and cylinder liner; comparison of lube oil samples before and after failure; examination of the water jacket around the 2L cylinder liner for water blockage; in-
- spection of other cylinder liners from the diesel crank-case and of the fuel metering rod for possible jamming; and testing of the 2L fuel injector.
The piston, rings, cylinder liner and engine lube oil were changed; and a check of the engine operation by the diesel's manufacturer representative was performed. The diesel generator was returned to service approximately five days fellowing the failure. This test is classified as valid in accordance with Regulatory Guide 1.108, Section c.2.e.(6), and is the
~ first valid failure in the last 100 valid tests. The current surveillance e
frequency remains 31 days in complisnce with Technical Specification k
Table 4.8.1.1.2-1 and Section C.2.d of Regulatory Guide 1.108.
