In accordance with ASME Section XI, Code Case N-532-4, "Repair/Replacement Activity Documentation Requirements and In-service Summary Report Preparation and Submission" Indian Point Nuclear Generating Unit No.2 (IP2) hereby submits 2012 Form OAR-i, Owners Activity Report for Inservice Inspections. This report describes relevant conditions that required evaluation and repairs and replacements to be performed during the period April 2010 through April 2012.

Item Descritlon Evaluation Depscription CR-IP2-2012-1997-During an ISI liquid penetrant examination of the integral attachment weld to support C-C/C3.20 SW ACH 216/Integral welded attachment ACH-216 identified a rounded indication located on the support trunnion adjacent the attachment weld.
9G Engineering evaluated the condition and the existing condition with the defect is acceptable.
Accessible Surface Areas of Electrical CR-IP2-2012-1760-During IWE inspections of Containment penetrations, water was observed seeping from E-A/E1.12 Penetrations an area adjacent to electrical penetration # 69 outside Containment at the 10 o'clock position relative to apenetration # 69. Engineering evaluated the condition and the condition was accepted as is.
A Class 1 Piping Supports - One Directional/
CR-P2-2012-2046-During an ISI VT-3 examination a bent hanger rod was observed creating a misalignmen F-F.1A Class 139 Pibetween the pipe attachment and the building structure. Engineering evaluated the condition and the CH 139 condition was accepted as is.
Class 1 Piping Supports - Multi-directional/
CR-IP2-2012-1901-During an ISI VT-3 inspection, the U-bolt to pipe clearance was found out of tolerance.
SR 916 Engineering evaluated the condition and the condition was accepted as is s1 Piping Supports - Thermal CR-IP2-2010-1926/CR-IP2-2010-2047 -The spring can settings were outside the range of the hot and cold F-A Class 1 Piping Spr 556 load setting specified on the support detail drawing.
