The Nuclear Regulatory Commission recently published a Federal Register Notice announcing our intent to submit an information collection request to the Office of Management and Budget (OMB) for NRC Form 396, Certification of Medical Examination by Facility Licensee (3150-0024). A supporting statement describing the requested information is available at http://regulations.gov by searching Docket No. NRC-2025-0011.

As a potential respondent to this information collection, we are interested in your feedback.

Specifically, we are seeking input on the following questions by November 3, 2025:

1. Is the proposed collection of information necessary for the NRC to properly perform its functions? Does the information have practical utility? Please explain your response.
2. Is the burden estimate accurate? Please explain your response.
3. Is there a way to enhance the quality, utility, and clarity of the information to be collected?
4. How can the burden of the information collection be minimized, including the use of automated collection techniques or other forms of information technology?
