Reactor Oversight Process 6
- Baseline Inspection Program
- Minimum level of inspection conducted at all facilities regardless of their performance
- Reactive Inspections (e.g., Special Inspections)
- Investigate events involving reactor facilities in a timely, objective, systematic and technically sound manner
- Supplemental Inspections (e.g., IP 95001)
- Apply NRC inspection resources in a graded manner when risk-significant performance issues are identified 7

Inspectors determined that inadequate work instructions used during maintenance in Oct. 2023 allowed the leak to develop.
Finding significance was assessed under IMC 0609 - White A supplemental Inspection (IP 95001) was recently completed.
The plant transitioned from the Regulatory Response to the Licensee Response Column effective January 30, 2025.

Transitioned to the Regulatory Response Column, July 1, 2024, due to the Unplanned Scrams per 7000 Critical Hours Performance Indicator crossing the Green-White threshold.
The Performance Indicator returned to Green in the 4th Quarter of 2024.
Unit 2 will transition to the Licensee Response Column when the Supplemental Inspection (IP 95001) is completed and the inspection report is issued.

Inspectors determined that there were multiple instances of work orders, operating experience, and industry recommendations that could have prevented the failure.
Finding significance was assessed under IMC 0609 - White Susquehanna will transition to the Licensee Response Column when the Supplemental Inspection (IP 95001) is completed and the inspection report is issued.
