Melfi. Jim; Strittmatter. Henry Comanche Peak Unit 2 Plant Trip Monday, June 7, 2021 5:40:00 PM Neil Day, resident inspector at Comanche Peak, was informed by the licensee that Unit 2 tripped due to a fire in main transformer 2MT1 at 1527 CDT. Neil is responding to the plant.
Unit 2 was at 100 percent power prior to the trip. The licensee stated that the plant responded as expected. The deluge system actuates and the fire brigade responded with a site fire truck and were able to put the fire out in 20 minutes.
No emergency was declared.
This transformer raises the output of the main generator up to grid voltage and is located outside the turbine building.
Please expect an update late today.

This is their testing plan for 2MT1. They are bringing 2 vendors (ABB and Primoris) onsite to perform the inspections.
They estimated about a day and a half for the inspections on 2MT1, and they haven't arrived onsite yet.

- Team, The residents arrived onsite to verify the plant was stable following the automatic reactor trip.
The Unit 2 Generator Lockout Relays were actuated due to the issue with the Unit 2 Main Transformer (2MT1 ). This caused the U2 Main turbine to trip, and since the Reactor was at 100% caused an automatic reactor trip (>50% Rx power turbine trip). The cause of the Transformer issue is under investigation (e.g. fault, grid lightning strike, etc.). There were no precursor alarms of note with respect to issues with the transformer.
Attached is a picture of the 2MT1 main transformer. Note the top center bushing is severely damaged. Pieces of this bushing were scattered in the yard area.
The control room received a report of the fire from individuals in the field at 1527 (concurrent with the automatic trip). The deluge suppression actuated, and the fire brigade responded. The fire was reported out at 1546.
The licensee did not declare any emergencies due to the fire. The residents reviewed the EALs and interviewed Operations Personnel. Based on the location and duration of the fire, that appears to be the right determination (HU4 in the EALs).
The residents will be following up on the licensee's investigations and repairs. They are currently in Mode 3 and have the OCC staffed.

Comanche Peak Unit 2 Transformer Update
- All, The licensee is still in an investigation effort to figure out what happened to cause the fire and the trip. Oil samples show normal conditions, so the transformer is probably not damaged internally. The licensee has ABB onsite to do 24-36 hours of testing with a goal of understanding whether the transformer can be repaired.
In parallel, the licensee is planning what would need to be done to replace both of the current transformers (it takes two matching transformers to carry the full generator output load). The spares on site were previously installed in Unit 2 but were replaced with the current ones to support the greater capacity when the unit was approved for a power uprate. This means that if the old transformers are installed, Unit 2 will not be able to run at full capacity (though the difference if not huge).
It is also possible to disconnect that problem transformer and run at 50-60% power while repairing or replacing the damaged transformer.
We should learn more tomorrow.

- Aaron, Q"Keefe Neil Mccraw. Aaron RE: Comanche Peak Unit 2 Path Forward Thursday, June 10, 2021 4:33:00 PM Both Comanche Peak units each have a pair of 3-phase transformers. It takes both to be able to put out the full electrical capacity the plant can generate.
If they do a temporary modification to remove the damaged transformer from service (and confirm the other transformer is undamaged, which is still being tested), they could run on the one remaining transformer and it could handle 50-60% of the full output.
Obviously, the peak power season in Texas is upon us, and they would not want to limit their output like that long-term, so this is not their favorite option. As of this morning, all the testing is not done yet, but none of the initial tests have caused them to rule out repairing the damaged transformer. They should be getting close to knowing if they can repair it Friday or Saturday.

Comanche Peak Unit 2 Path Forward Hi Neil, Forgive me for not being to attend Region IV's morning safety meeting this morning. I was just reviewing the notes and read the last line: "One option will require derating the unit and the other will limit power to 50-60%." The derating option, correct me if I'm wrong, is installing a spare, but older transformer at the site (Scott mentioned this at another meeting yesterday). What would the "limit power to 50-60%" option entail?
