It looks fine, i.e., adequate. As far as where and when the form 314 are mentioned, the only successful Complex Materials Decommissioning license termination that I have been connected to, was the Hematite Site. The FRN, attached, mentions the Form 314 exactly once just before the signature. The transmittal letter mentions it once and the SER mentions it twice.
A previous license termination of the Mallinckrodt, LLC facility in 2016 does not mention the 314 in the FRN, attached, nor the SER; however, it is mentioned in the Letter transmitting the SER and amended terminated license to the licensee, attached.
