Form 4.1-1 Record of Rejected Knowledge and Abilities Refer to Examination Standard (ES)-4.2, Developing Written Examinations, Section B.3, for deviations from the approved written examination outline.
Tier/Group Randomly Selected K/A Reason for Rejection Q12 T1/G1 295024 EK2.01 Could not determine a link between High Reactor Pressure and the Plant/process computer that would not be considered minutia at Fermi 2 or would not be LOD 1.
Recommend keeping 295025, High Reactor Pressure, which is the same EPE on the sample plan as the originally selected EPE.
Please consider EK2.08 (same K2 column) for a replacement. K2.08 is Reactor Pressure Regulating System.
NRC: 268024EK2.08 selected as replacement K/A Q43 T2/G1 223002 K5.02 No clear link could be found between the Primary Containment Isolation System (PCIS) and Secondary Containment Integrity. Any question written to this concept would likely be called out as being minutia during exam review if one could be written at all.
Recommend keeping 223002, Primary Containment Isolation System, which is the same on the sample plan as the originally selected system.
Please consider K5.01 (same K5 column on the sample plan) for a replacement. K5.01 is Primary Containment Integrity.

Q52 T2/G1 510000 K5.03 Nothing could be found related to Service Water System and Flood prevention. Some actions in the AOP for flooding could be found, that relate to service water systems, but nothing that would be considered systems knowledge and therefore nothing appropriate for Tier 2 of the sample plan.
Recommend keeping 510000, Service Water System, which is the same on the sample plan as the originally selected system.
Please consider K5.01 (same K5 column on the sample plan) for a replacement. K5.01 is Intake Travelling Screen high differential pressure /
differential level.
NRC: 510000K5.01 selected as replacement K/A Q58 T2/G2 230000 K4.04 Validators said this concept is not operationally valid. It is not possible to over-pressurize RHR piping from the Torus/Suppression Pool Spray Mode. Could not write a question to hit this K/A that would be operationally valid.
Request a new K/A for system 230000. Request K4.03 as a replacement.

T2/G2 201002 A2.03 Several attempts were made for this K/A. They were all too AOP heavy, which made them poor questions for Tier 2. Also, the validators classified the question LOD as 5 (minutia) due to it testing too deep into the AOP that the question was written against.
Request a new system: 204000 SF2 RWCU Reactor Water Cleanup System.
Request new K/A 204000 A2.11, Ability to (a) predict the impacts of the following on the Reactor Water Cleanup System and (b) based on those predictions, use procedures to correct, control, or mitigate the consequences of those abnormal operations:
Abnormal system flow (SRO IR 3.1).
NRC: 204000A2.11 selected as replacement system and K/A.
Q02 T1/G1 295003 AK3.08 Several attempts were made at this K/A. The most recent attempt was validated and all 3 validators missed the question and all 3 selected 3 different responses. During the exam review process, the validators said the question was LOD=5 (minutia) and not operationally significant to know the information from memory. Therefore, Fermi 2 requests the following:
Maintain the same APE: 295003 Partial or Complete Loss of AC Power.
Request new K/A AK3.01 (Knowledge of the reasons for) Manual and automatic bus transfer.
NRC: Upon further review of the question and provided references, which were not available during early review of the question, the NRC has approved the swap to AK3.01, Manual and automatic bus transfer.
The K/A has been swapped due to the verbiage in the stem of the question not aligning with the actions performed in the procedure referenced. When all Subsequent Actions have been performed per the procedure, having the excessive cooldown rate occur afterwards is not realistic and creates an operational validity concern. Step J has the loss of Bus 72R corrected and the Action states to restore power to Bus 72R.
With Bus 72R restored, the valves in question would no longer be affected and the excessive cooldown is questionable.
Based on the amount of time already put into this question, the NRC has decided that it would be most beneficial for the exam development process to grant the change in the K/A rather than correcting this question which has already been Keep in mind, questions not validating at a high pass rate does not constitute a reason to swap out a K/A. The question and information contained in the question does not meet minutia level. The information being tested was considered system level knowledge and is a Note in the AOP and therefore is considered questionable material for the RO portion of the exam.
Q72 T4 292004 K1.15 Several attempts were made to write a question to this K/A. a version was submitted to the NRC that was called Unsat due to not meeting the intent of the K/A. The major issue with this K/A is how similar the two terms Reactivity Coefficient and Reactivity Defect are. When attempting to write a question to this K/A, validators expressed difficulty in picking out the differences on a multiple choice exam and expressed that this concept would be better suited for an essay type question, since the two terms are very similar. Also, it was extremely difficult to come up with two other plausible distractors.
Because of the extensive amount of time spent on this K/A, Fermi 2 requests a new K/A be selected:

Please replace K1.15 with K1.14, Compare the relative magnitudes of the temperature, Doppler, and void coefficients of reactivity.
Q77 (SRO)
T1/G1 295006 AA2.05 Submitted question was called Unsat by the NRC. Several attempts were made to write a question for this K/A but an acceptable SRO-Only version could not be written.
