Significant Violations
- NOV 07003103/2022007-01, Failure to meet performance requirements in 10 CFR 70.61(b) by not establishing IROFS barriers (Performance Area: Safety Operations)
These two NOVs are Severity Level II violations, that the NRC considered to be safety-significant issues during the LPR. As a result, the NRC identified an Area Needing Improvement (ANI).
15
Additional Violations
- NCV 07003103/2022001-01, Failure to make a required report
- NCV 07003103/2022003-02, Failure to Conduct Adequate Exercise Critiques
- NCV 07003103/2022401-01, Failure to conduct adequate material control and accounting investigation
- NCV 07003103/2023003-01, Lack of Implementation of Safety Controls for an Analyzed High Consequence Event
- NCV 07003103/2023003-02, Corrective Actions not Aligned with Causes in Root Cause Evaluation 16
2023 Program Adjustment
- On August 24, 2023, the NRC issued a Program Adjustment in area of Safety Operations.
- Added ~45 hours of Operational Safety inspection to an already scheduled core inspection the week of August 28.
- This inspection focused on corrective actions from the SL-II violations, particularly as they were applied to other IROFS50 series controls.
- These inspections resulted in two NCVs, documented in Inspection Report 2023-003 (ML23289A004), and listed previously.
