SUBJECT: REQUEST FOR ADDITIONAL INFORMATION
Dear Cody McClane:
The Nuclear Regulatory Commission (NRC) has completed the technical review of the renewal
application dated February 27, 2024 for license number 50-21374-01, and additional information
is needed. Please provide the response on a signed and dated letter, in company letterhead,
within 30 days from receipt of this letter, and make reference to mail control number 639867.

Please confirm that you want authorization for temporary job sites within U.S. NRC
jurisdiction.
2.
Please provide an organizational chart or structure for 49-50.
3.
In your application, you provided the gauge manufacturer and model number. Please
provide the radionuclide and specific activity per source and number of gauges you are
requesting. If you have an inventory sheet, please provide that as well.
In accordance with 10 CFR 2.390 of the NRCs Rules of Practice, a copy of this letter will be
available electronically for public inspection in the NRC Public Document Room or from the
NRCs Agencywide Documents Access and Management System (ADAMS). ADAMS is
accessible from the NRC Web site at http://www.nrc.gov/reading-rm/adams.html.
