Re: Business Entity: PILGRIM NUCLEAR POWER STATION Injured Party: Adam Snyder Loc.of Incident: 600 Rocky Hill Road, Plymouth, MA 02360

Dear Sir or Madam:

This is a request under the Freedom of Information Act pursuant to 5 U.S.C., § 552. Kindly forward to our office any and all OSHA records related to the above-referenced incident. Please include, without limitations:

1. Complete copies of any and all complaints, investigate reports, witness statements, memoranda, correspondence, records, and/or other documents which refer or relate to an incident where workers c ame in contact with airborne radiation during a phase of the decommissioning in August of 2020, at the work site located at the above -referenced location of 600 Rocky Hill Road, Plymouth, MA 02360.

2. Complete copies of any and all citations, fines, penalties, administrative decisions, and /or other documents pertaining to any and all vi ol ations alleged or determined to have been committed in connection with the above-referenced incident.

3. Complete color prints of any and all photographs inU.S.N.R.Cs possession, custody or control concerning, referring or relating to the above -referenced incident.

4. Complete copy of any videotape and/or DVD disc concerning any filming in U.S.N.R.C.'s possession, custody or control concerning, referring or relating to the incident referenced above.
