EIP-ZZ-00201 ADDENDUM C Rev. 039 Page 2 of 9 INFORMATION USE EOF NOTIFICATION PACKAGE Prepare for EOF Notifications Turnover from Control Room NOTE The goal is to complete Steps 1 - 9 within 90 minutes (assumes 15 minutes from arrival at the facility) from the time of the declaration.

Actions on this page are initial set-up steps to verify equipment is operational and prepare for turnover from the Control Room Communicator and should only need to be completed once.

If the EOF is uninhabitable, communications should remain with the Control Room Communicator until the backup EOF (BEOF) is staffed.

IF the EOF becomes uninhabitable after assuming responsibilities, TRANSFER responsibility for notifications using EIP-ZZ-C0010 Addendum G.

Sign in on Facility Sign-in board.

2.

Inform the Recovery Manager of your presence.

3.

Clip on Communicator badge.

4.

Initiate Facility Log sheet.

5.

Check fax and Event Notification System printer for communications, and provide copy to RM.

6.

Ensure Event Notification System PC is powered on and Event Notification System application is available.

7.

Ensure Backup Radio (BURS) is powered on and "REPTR-25, EOC=BURS" is displayed. If needed, toggle radio power on and off to restore correct display.

8.

Check Communicator phone lines for dial tone.

9.

When steps 1 - 8 are complete:

  • Inform the RM you are ready to accept Off Site Notification responsibility.
  • Record time ready to accept Off Site Notification in Facility Log Sheet.
  • Begin timer counting down to 30 minutes since last notification.

PERFORM ONLY IF Logistics personnel are NOT available.

Turn on projected status board.

Adjust Gaitronics to an acceptable level.

NOTE Initial notifications (e.g., a NEW Event Classification or a NEW or MODIFIED Protective Action Recommendation (PARS)) are required to be made AND ACKNOWLEDGED in 15 minutes. Follow up notifications are made approximately every 60 minutes.

EIP-ZZ-00201 ADDENDUM C Rev. 039 Page 3 of 9 INFORMATION USE TURNOVER FROM CONTROL ROOM - ASSUMING EOF NOTIFICATIONS NOTE Once the Event Notification System notification are accepted by a facility, no other facility should send an Event Notification System notification.

1. Review the most recent Event Notification System/fax notification from the Control Room.
2. Contact the CR Communicator for a brief via NRC bridge line (9-1-301-816-5100). Provide your position (EOF Communicator), your name, and that you are ready to start turnover.

6. Upon direction of the RM, advise CR Communicator that the EOF is assuming responsibility for Off-Site Notifications including:

Complete the Event Notification System screen using information provided by Recovery Manager (RM), Protective Measures Coordinator (PMC), and Dose Assessment Coordinator (DAC).

Obtain Plant and Emergency Status from the RM.

Obtain Radiological Release Information from the DAC.

Obtain Plant Conditions and Protective Action Recommendations from the PMC.

Prior to sending the Event Notification System notification, Obtain approval from RM to send.

Document acknowledgement of notification. If a county does NOT respond as indicated by Event Notification System Tracking, continue with contact methods until all counties respond. Request AND document acknowledgement. Acknowledgement from either EOC or 911 of a given county constitutes a response.
