60-day report to January 2, 2023, incident reported under 10 CFR 70 Appendix A Concurrent Report (NRC Event 56292); Framatome Richland Facility; License No. SNWI-1227; Docket No. 70-1257

On January 2, 2023, the Framatome Richland facility reported to the NRC Operations Center that a Framatome employee preparing an area for painting experienced a hand injury when a heavy object fell on his hand. The worker was surveyed out of the area and confirmed to be free from radioactive contamination above free release limits and was transported to the local hospital (Kadlec Regional Medical Center) for treatment.

The Washington State Department of Labor and Industry (L&l) was notified of this injury on 01/02/2023 at 16:28. The work area was quarantined pending investigation by L&l. L&l completed their investigation of the accident scene on January 18, 2023. On February 3, 2023, L&l issued an inspection report to Framatome and reported We found no safety violations at this sit during our visit and have not issued any violations or penalties.

The initial report to the NRC Operations Center was made per the requirements of 10 CFR 70 Appendix A (C) Concurrent reports because the nature of injury required reporting to L&l.

The condition was reported to the NRC Operations Center by Calvin Manning, Framatome Manager of Licensing and Compliance, on January 3, 2023, at 12:47 ET. Mr. Manning s telephone number is (509) 375-8237.

Date, Time, and Exact Location of Incident

The reportable injury occurred on January 2, 2023, at approximately 12:20 FT. The location was in the ELO Building of Framatome s Richland facility at 2101 Horn Rapids Road, Richland WA 99354.

Incident Description

On January 2, 2023, at approximately 12:20 a Framatome employee preparing an area for painting experienced a hand injury when a heavy object fell on his hand.

Health and Safety Consequences

The injured employee was transported to a local hospital and was treated for the injury.

External Conditions

No external conditions were identified.

Status of the Event

The injured employee has since returned to work, the work area has been released by L&l and the work activities in the area have been returned to normal.

Notifications to Other Agencies

Washington State Department of Labor and Industry was notified of the injury on 01/02/2023 at 1628.

Inclusion in Integrated Safety Analysis

NA. This condition relates to an industrial injury.

Probable Causes of Event

This injury occurred because a heavy object was not completely dismantled and removed during a D&D activity. The worker was unaware that the heavy object was not adequately secured to prevent falling and his work activity caused sufficient vibration to cause the object to fall and land on his hand.

Corrective and Mitigating Actions

Framatome had the employee transported by ambulance to the local hospital for treatment.

Once the work area was released by L&l, the work area was inspected and verified to be in a safe configuration before allowing work to proceed in this area.

Framatome conducted safety standdowns in all work areas to review this injury and its causes.

Framatome employees also completed safety walk downs in all work areas looking for other heavy objects that could fall and injure workers. Any heavy objects identified during the framatome

walkdowns that could fall and cause an injury were relocated to reduce the risk of similar injuries in the future.

Preventive Actions

Framatome trained plant engineers and craft/craft leadership regarding situational awareness and questioning attitude while in the Line of Fire.

Review procedures that cover project/job site transition activities. Make procedure changes, as appropriate, for gate reviews, PJB s (when jobs extend to multiple days, and turnover meetings between organizations or within organizations (craft to craft). Verify procedures adequately emphasize stopping work if the area is cluttered or if equipment is not in a safe state for work to continue.

Review monthly housekeeping/fire safety PM procedure and management expectations with PM performers. Emphasis heightened awareness for potential Line of Fire accidents and Extent of Condition reviews of events that have occurred since the last inspection (these are typically covered in Site Status Slides following events). Update, as appropriate Housekeeping PMs list Line of Fire situations as conditions to look for.

Re-train plant engineers and craft/craft leadership regarding situational awareness and questioning attitude relative to potential Line of Fire accidents
