Dear Sir or Madam,

Pursuant to 10 CFR 71.95(a)(3), Framatome Inc. (Framatome) submits this report for self-identification of instances where Framatome made multiple shipments of unirradiated fuel assemblies which the conditions of approval in the Certificate of Compliance (CoC) were not observed in making a shipment for the NRC-approved Type B package. Framatome is the certificate holder for this packaging.

On September 15th, 2022, a single TN-B1 packaging (USA/9372/B(U)F-96) was found to be missing a single non-structural component of the outer containers vibration isolation system. This TN-B1 packaging, serial number RA-021, was undergoing refurbishment when the missing component was identified. It is unknown when this component was initially missing. The missing component is in the licensing drawing FS1-0042700, Item 3, Bolt Holder.

Framatome is currently confirming the correct configuration of the vibration isolation system within our entire fleet and have worked through 200 out of 250 total packagings.

To date, this non-conformance is only associated with a single vibration isolation assembly within the RA-021 outer container.

The attachment to this letter provides the required details for the report.

10 CFR 71.95 - Type B Transportation Package Report (USA/9372/B(U)F-96)

1. Brief abstract describing the major occurrences 71.95(c)(1)

The TN-B1 package is used to transport unirradiated fresh fuel assemblies, both channeled and unchanneled, with a U-235 mass percentage not to exceed 5 weight percent.

The drawings identified in the Certificate of Compliance, USA/9372/B(U)F-96, Revision 3, includes in section 5(a)(3), drawing FS1-0042700, Rev. 1. The Item 3, Bolt Holder, was identified as missing at one of the eight locations within the outer container of packaging serial number RA-021 on September 15th, 2022.

The purpose of this Item 3 component is to assist with holding the Items 4 and 5, Bolt and Pin for Vibroisolating Rubber parts during assembly of the vibration isolation components.

There was no component or system failure that contributed to this event.

2. Narrative description of the event, 71.95(c)(2)

(i) Status of components or systems that were inoperable at the start of the event and that contributed to the event;

The Item 3 Bolt Holder is used as an aid during assembly of the vibration isolation components. There were no inoperable components or systems.

(ii) Dates and approximate times of occurrences;

The Item 3, Bolt Holder, from drawing FS1-0042700 was identified as missing at one of the eight locations within the outer container on September 15th, 2022. It is unknown when the Item 3 Bolt Holder was removed. It is possible that this component was not installed during the original manufacture in 2004.

(iii) The cause of each component or system failure or personnel error, if known;

The cause of the missing Item 3 Bolt Holder component is unknown. All eight locations of this component are in a location that is not visible without disassembly of the outer container and therefore, not visually inspected during regular packaging refurbishment.

(iv) The failure mode, mechanism, and effect of each failed component, if known;

There is no effect to the packaging due to this missing component. The purpose of the Item 3, Bolt Holder component is to aid in the assembly of the vibration isolation components by specifically supporting Items 4 and 5, Bolt and Pin for Vibroisolating Rubber parts of drawing FS1-0042700.

(v) A list of systems or secondary functions that were also affected for failures of components with multiple functions;

No systems or secondary functions were affected for failures.

(vi) The method of discovery of each component or system failure or procedural error;

The non-conforming component was identified by Framatome during refurbishment activities and replacement of the Item 9, Vibroisolating Rubber components, identified on drawing FS1-0042700.

(vii) For each human performance-related root cause, a discussion of the cause(s) and circumstances;

The root cause for the missing component is not known. It is possible that the component was not installed by the original packaging manufacturer when supplied in 2004.

(viii) The manufacturer and model number (or other identification) of each component that failed during the event;

There were no component failures.

(ix) For events occurring during use of a packaging, the quantities and chemical and physical form(s) of the package contents.

The TN-B1 packages (USA/9372/B(U)F-96) may be shipped with either one or two unirradiated fuel assemblies or up to 130 loose rods. Solid uranium oxides or uranium carbide fuel rods enriched to no more than 5.0 weight percent U-235.

The proper shipping name typically used is: UN3328, Radioactive Material, Type B (U) Package, Fissile, Class 7, (2.2). The contents were solid uranium dioxide in fuel assemblies, uranium enriched to 20% or less. The non-flammable gas subsidiary hazard was for the pressurized helium gas within the fuel rods.

3. Assessment of Safety Consequences and Implications of the Event, 71.95(c)(3)

There is no failure and no safety consequence to the missing Item 3, Bolt Holder component. A replacement component was installed without issue during refurbishment of RA-021.

4. Corrective actions taken, 71.95(c)(4)

Framatomes fleet of TN-B1 packages are being fully inspected during the regular refurbishment schedule. To date, 200 out of 250 packagings have been specifically inspected for this condition with no non-conformances identified.

There are no previous or similar events identified with this non-conformance.

No individuals were exposed to radiation or radioactive material due to this event.

There was no leakage of contents due to the non-conformance and no reduction in the performance of the packaging.
