THESE CABLES WERE RESTORED TO AN OPERABLE STATUS WITHIN APPROXIMATELY 2 HOURS OF THE DISCOVERY OF THEIR LOSS WHICH RETURNED THE 10 EMERGENCY OFFSITE SIRENS TO A FULLY OPERABLE STATUS. BACKUP MANUAL ACTUATION CAPABILITY FOR THESE 10 SIRENS WAS AVAILABLE. THE LICENSEE REVIEWED THE REPORTABILITY OF THIS EVENT ON 02/13/90 AND DETERMINED THIS NOT TO BE REPORTABLE UNDER FEMA GUIDELINES. HOWEVER FURTHER REVIEW PERFORMED ON 02/14/90 IT WAS DETERMINED THAT THIS CONSTITUTED AND EVENT UNDER 10 CFR 50.72 b.1.v. THEY KNOW THAT EMPLOYEES AT CAMP PENDLETON MARINE BASE WERE WORKING ON CABLES THAT GO INTO THE BUILDING WHICH CONTAINED THE CONTROL PANEL FOR THE 10 EMERGENCY SIRENS THAT WERE INOPERABLE. LICENSEE NOTIFIED REGION 9 FEMA REPRESENTATIVE. RESIDENT INSPECTOR WILL BE INFORMED BY THE LICENSEE.

BETWEEN 0850 PDT AND 1515 PDT LICENSEE HAD FIVE INOPERABLE OFFSITE SIRENS. PER THEIR PROCEDURES THEY HAVE TO NOTIFY NRC AND STATE THAT THEY FIVE OR MORE INOPERABLE OFFSITE EMERGENCY SIRENS. LICENSEE DID NOT KNOW HOW THE SIRENS BECAME INOPERABLE AND LICENSEE DID NOT KNOW WHAT HAD BEEN WRONG WITH OFFSITE EMERGENCY SIREN #

FOUR WHEN THEY RETURNED IT TO SERVICE AT 1515 PDT. LICENSEE SAID THAT THE TELEPHONE COMPANY IS REPAIRING THE FOUR OUT OF SERVICE OFFSITE EMERGENCY SIRENS AT THIS TIME.

RESIDENT INSPECTOR WILL BE INFORMED BY THE LICENSEE.

THE CITY OF SAN CLEMENTE's EMERGENCY COORDINATION RECEIVED SOME PHONE CALLS AND SUBSEQUENTLY ISSUED A PRESS RELEASE REGARDING THE INADVERTENT ACTUATION. THE LICENSEE HAS NOTIFIED THE RESIDENT INSPECTOR.

POWER WAS RESTORED TO THE SIRENS AT 0210 (PDT) ON JULY 31, 1992. TWO OTHER SIRENS WERE OUT OF SERVICE AT THE TIME DUE TO UNRELATED REPAIR ACTIVITIES. THE TOTAL OF 5 INOPERABLE SIRENS FOR A PERIOD OF 3 HOURS AND 18 MINUTES MET THE LICENSEE'S REPORTING CRITERIA OF 5 INOPERABLE SIRENS FOR GREATER THAN 1 HOUR.

THE CONDITION WAS NOT REPORTED TO OPERATIONS PERSONNEL UNTIL AFTER THE POWER HAD BEEN RESTORED TO THE THREE DEENERGIZED SIRENS. AT THAT TIME THE LICENSEE DETERMINED THAT THE EVENT WAS REPORTABLE.

THE LICENSEE HAS NOTIFIED THE RESIDENT INSPECTOR.

THE LICENSEE CONTACTED THE LOCAL PHONE COMPANY TO INVESTIGATE THE REASON FOR THE ALARMS. THE PHONE COMPANY REPORTED THAT CONSTRUCTION WAS BEING CONDUCTED IN THE AREA OF EMERGENCY SIREN TELEPHONE CABLES AND THAT THE CABLES HAD BEEN ACCIDENTALLY CUT.

A FIFTH EMERGENCY SIREN HAD ALREADY BEEN DECLARED INOPERABLE BY THE LICENSEE. THE LICENSEE IS REQUIRED TO REPORT INOPERABLE SIRENS UNDER 10CFR50.72 WHEN THE NUMBER OF INOPERABLE SIRENS REACHES FIVE.

THE TELEPHONE COMPANY HAS REPAIRED THE CABLES. THE LICENSEE DECLARED THE FOUR SIRENS OPERABLE AT 1225 PST.

THE LICENSEE INFORMED THE NRC RESIDENT INSPECTOR.

THE FIRST TROUBLE LIGHT INDICATION WAS NOTICED AT 1349 PST FOLLOWING TROUBLE LIGHT INDICATIONS WERE NOTICED AT 1533, 1547, 1558 AND 1603.

THE LICENSEE WILL INFORM THE RESIDENT INSPECTOR.

AT 19:30 HOURS, PDT, ON JANUARY 17, 1993, THE SONGS TELECOMMUNICATIONS DEPARTMENT DETERMINED THAT 10 COMMUNITY ALERT SIRENS LOCATED ON CAMP PENDLETON WERE INOPERABLE.

SCE HAS DEFINED A SIGNIFICANT PORTION OF THE OFFSITE NOTIFICATION SYSTEM TO BE EITHER 16 COMMUNITY ALERT SIRENS INOPERABLE FOR GREATER THAN ONE HOUR, OR FIVE (5) COMMUNITY ALERT SIRENS INOPERABLE FOR GREATER THAN 48 HOURS. THE SIRENS ARE NOT EXPECTED TO BE PLACED BACK IN SERVICE PRIOR TO EXPIRATION OF THE 48 HOUR LIMIT AT 19:30 THIS EVENING. THUS, THIS REPORT IS AN EARLY ONE-HOUR TELEPHONE NOTIFICATION.

THE CAUSE HAS BEEN ATTRIBUTED TO ADVERSE WEATHER CONDITIONS (RAIN). REPAIRS ARE EXPECTED TO BE COMPLETED WITHIN ONE WEEK AND IS DEPENDENT ON CAMP PENDLETON (USMC) TELEPHONE SYSTEM STORM DAMAGE REPAIR CAPABILITIES. CAMP PENDLETON RETAINS EMERGENCY RESPONSE CAPABILITY THROUGH THE USE OF THEIR CHAIN OF COMMAND SYSTEM.

SCE WILL NOTIFY THE NRC ONCE THE NUMBER OF INOPERABLE SIRENS IS REDUCED TO LESS THAN FIVE.

UNIT 1 CURRENTLY SHUTDOWN IN MODE 5, UNIT 2 IS AT 100%, AND UNIT 3 IS IN MODE 2 AT 2.5% POWER.

THERE WAS NO MEDIA INTEREST IN THE EVENT. A COURTESY NOTIFICATION WAS MADE TO FEMA (MR.

ALL 17 SIRENS HAVE BEEN SECURED FOR REPAIRS, WHICH IS EXPECTED TO LAST 3 OR 4 HOURS. THE LICENSEE HAS NOTIFIED THE RESIDENT INSPECTOR.

"This notification is for San Onofre Units 1, 2, and 3 and is being made in accordance with 10CFR50.72(b)(2)(vi) for an event related to the public health and safety for which a news release is planned or notification to other government agencies has been or will be made. Southern California Edison (SCE) is providing this information to the NRC due to possible questions the NRC may receive from the public."

"On September 29, 1999, at 1019 PDT, SCE initiated a test of the offsite sirens. All local residents, businesses, and schools were notified of the test beforehand. At about 1025 PDT, SCE was notified that a local cable [television] station had broadcast an actual emergency evacuation message to local residents. That erroneous message has been corrected, and all local media outlets have been notified not to pick up the incorrect report."

"[... ] At the time of this occurrence, Unit 1 remained permanently defueled, and Units 2 and 3 were operating at about full power."

The licensee stated that the cause of the erroneous message is currently under investigation. The licensee also stated that the site received a couple of calls from members of the public and that 911 received over 100 calls as well.

The licensee notified the NRC resident inspectors, the emergency preparedness staff at NRC Region 4, and the NRC NRR project manager for San Onofre Units 2 and 3 (L. Raghaven). NRC Region 4 plans to notify the State of California and the applicable Federal Emergency Management Agency's regional office (FEMA Region 9).
