Local, Protected Area or Site Evacuation Page: 4 of 20 1.0 PURPOSE This procedure provides direction for the Shift Manager or individuals perfom,ing Emergency Director Duties to conduct Localized Evacuations within the Protected Area (PA}, Protected Area Evacuations and Site Evacuation of non-essential personnel, including the Industrial Development Complex (IDC). {R-5.1}

2.0 DISCUSSION 2.1 This procedure outlines the overall processes for conducting evacuations to protect plant personnel. Individual positions actions are provided in this procedure and in their Position Specific Checklist.

2.2 The principle consideration when contemplating an evacuation is the safety of personnel.

Depending on conditions at the time, the Shift Manager/ Emergency Director may direct one of three evacuations:

2.2.1 A Localized Evacuation is the orderly withdrawal of personnel from a selecte<iarea.

A Localized evacuation will be announced using the alerting tone followed by1m instructional message.

2.2.2 A Site Evacuation encompasses the Columbia Generating- station Protected Area and the Exclusion Area, including all of the Owner Controlled Area (OCA). A Site evacuation normally occurs at Site Area Emergency, and non-essential personnel from the Protected Area, Exclusion Area, and the Industrial Development Complex are evacuated.

2.2.3 A Protected Area evacuation is the orderly withdrawal of personnel from the _.J_

Protected Area resulting from a security threat or Emergency Director discretion.

A Protected Area Evacuation may be conducted in response to a Hostile Action, a validated report of Airborne Threat, or a confim,ed, credible insider threat. This evacuation may result independent of emergency classification. Confim,ation-of this threat and the recommendation to either "Take Cover", assembly or evacuate should come from On-Shift Security Supervision. The determination to "Take Cover" or evacuate will be evaluated against the nature of the threat at the time. Provided -

a Protected Area Evacuation can be conducted safely during this event, evacuation is preferred. Otherwise, "Take Cover" is preferred. All Protected Area personnel should immediately follow public address announcement instructions.

2.3 "Take Cover" is a protective action taken when directed by plant announcements from Operations or Security, when evacuation cannot be perfom,ed safely due to security conditions. (This was sometimes previously referred to as "Shelteringn or "Sheltering in Place.".)

3.1 Emergency Director (Shift Manager, TSC Manager or EOF Manager)

The Emergency Director is responsible for determining the need for onsite protective actions (evacuation, sheltering, assembly, etc.) and directing necessary protective actions and public address announcements.

3.2 TSC Manager (additional duties)

The TSC Manager is responsible for coordinating appropriate public address announcements to be made from the TSC and establishing accountability with assistance from OSC staff within 30 minutes of initiation of an announcement to evacuate.

3.3 On-Shift Security Supervisor/ EOF Security Manager 3.3.1 The On-Shift Security Supervisor is responsible for advising on action*s that affect

  • implementation of the Safeguards Contingency Plan. These actions~nclude, but are not limited to, response to a Hostile Action within the PA or Owner Controlled Area t

,_i;j, (OCA), including a credible insider threat, and directing "Take Cover," Localized, or Protected Area evacuation in response to a security event.

3.3.2 If the Security Communications Center (SCC) is unable to activate the Site Evacuation siren from the sec, the On-Shift Security Supervisor, untit'relieved by the EOF Security Manager, is responsible to dispatch an officer to the siren to-:

activate it locally. Ongoing Hostile Action may delay this activity.  :\;

3.3.3 Ensuring actions necessary to evacuate the IDC are implemented. The On-S~ift Security Supervisor performs these actions until relieved by the EOF Security Manager.

3.3.4 Coordinate with Department of Energy-Richland (DOE-RL) authorities if evacuation route requires access beyond the Wye barricade (e.g., uses Route 11A).

3.3.5 In conjunction with Radiation Protection Manager (RPM), evaluate the need to relocate security staff from Bullet Resistance Enclosures (BREs) or other security positions .

Local, Protected Area or Site Evacuation Page: 6 of 20 3.4 Radiation Protection Manager The Radiation Protection Manager (RPM) is responsible for dispatching Health Physics (HP)

Technicians to the Protected Area Access Point (PAAP) to assist evacuees with personnel monitoring and. provide direction to evacuees to the Offsite Assembly Area for additional monitoring and if necessary, decontamination. This action may not be appropriate during Hostile Actions in the PA or OCA. The RPM also advises the Radiological Emergency Manager (REM) if personnel or vehicle monitoring or decontamination is necessary.

3.4.1 In conjunction with the On-Shift Security Supervisor/ EOF Security Manager, evaluate the need to relocate security staff from BREs or other security positions.

3.5 Radiological Emergency Manager The Radiological Emergency Manager (REM) is responsible for requesting HP Technician support to set up the Offsite Assembly Area for evacuee reception, directing personnel*and vehicle monitoring, and decontamination support. If a radiological hazard was not pre~nt and evacuees are not contaminated, the REM authorizes evacuees to be released from the*Offsite Assembly Area .

  • 3.6 Site Support Manager 3.6.1 The Site Support Manager is responsible for contacting the Department of Energy-Richland (DOE-RL) Emergency Operations Center (EOC) to provide advance notice of Columbia Generating Station protective action decisions for consideration.

3.6.2 Arranging transportation for any onsite indMduals who may not have availab~~*

means of evacuating the site.

4.0 PROCEDURE NOTE: Protective Actions for site personnel as outlined in this procedure are coordinated between the Shift Manager, TSC Manager and EOF Manager.

The Emergency Director is responsible for leading this coordination.

4.1 Determining Appropriate Action 4.1.1 IF immediate "Take Cover" is the preferred option based upon security request, THEN make PA announcement for personnel to take cover.

NOTE: Localized evacuations that may be conducted without being in a declared emergency condition, the decision to evacuate is the responsibility of the Control Room Supervisor/Shift Manager (CRS/SM) .

a. IF conditions in a specific area place plant personnel In immediate danger, such as:
  • Dangerous Atmosphere (Steam Leaks, Toxic. Lack of Oxygen)
  • Physical Hazards (falling objects or falling / tripping dangers)
  • Fire or explosion (actual or potential)
  • Radiological Conditions
  • Security issues
  • Shift Manager I Emergency Director Discretion THEN a Localized Evacuation shall be conducted.
b. IF a Localized Evacuation is determined to be required, THEN follow guidance in section 4.2 of this procedure to conduct evacuation.

4.1.3 Protected Area Evacuation

NOTE: A Protected Area Evacuation may be conducted in response to a Hostile Action, a validated report of Airborne Threat, or a confirmed, credible inside'f',threat.

I However, Security may request "Take Cover: Protected Area Assembly, or evacuation based on security conditions.

The Shift Manager, TSC Manager or EOF Manger may evacuate the Protected Area at their discretion. This may be done for degrading plant conditions at an Alert or conditions where non-essential personnel inside the Protected Area may impact emergency response. --c.

a. IF advised of an aircraft threat to Columbia,

THEN refer to ABN-AIRBORNE-ATTACKFLOWCHART for guidance on Plant Announcements.

b. IF Security reports a Hostile Action within the PA or OCA, or there is a confirmed credible Insider Threat, THEN determine appropriate actions as follows:

Confer with the On-Shift Security Supervisor to determine appropriate actions.

Protective action options include:

  • Protected Area Evacuation
  • "Take Cover"
  • Assembly inside the Protected Area
c. IF the decision is to Assemble inside the Protected Area, THEN direct personnel to assemble at the OSC/TSC on the West side of the plant (or other area as specified) .

d. IF personnel are reporting to the OSC/TSC, THEN have them card in at the TSC card reader for accountability purposes.
e. IF Protected Area Evacuation is called for, THEN conduct evacuation per Section 4.3 of this proc,edure.

4.1.4 Site Evacuation

a. A Site Evacuation is required for events classified as a Site Area Emergency or General Emergency, unless the movement of personnel will place them in more danger than retaining them onsite (such as Hostile Action within the PA or OCA, weather conditions, or other hazards preventing movement of personnel offsite).
b. The Emergency Director may call for a discretionary Site Evacuation based on de-grading plant conditions or other conditions where evacuation of non-essential personnel will improve emergency response actions. *1*

C. Determine if any of the extenuating conditions listed below are present, ofbther conditions which might preclude or delay safe evacuation:

  • An ongoing Hostile Action within the PA or OCA, or a credible security threat within the PA (consult with the On-Shift Security Supervisor to aid in determining the safest course of action).
  • IF event involves an airborne attack, THEN refer to the ABN-AI RBORNE-ATTACKFLOWCHART for guidance on evacuation and Plant Announcements. *
  • Inclement weather (e.g., high winds or hazardous road conditions may 1' preclude a safe evacuation of plant personneQ.
  • Radiological hazards exist (determine which action would result in lowest dose to evacuating personnel).
  • Other hazards exist which might subject evacuees to a higher risk to personnel safety than not evacuating
d. IF Site Evacuation is called for and can be safely conducted, THEN perform evacuation per section 4.4 of this procedure.
e. IF Site Evacuation is called for and CAN NOT be safely conducted, I!::1.£.M take appropriate alternate actions to protect onsite personnel and conduct evacuation when conditions permit. Alternate protective actions may include localized or Protected Area evacuation, onslte assembly, shelter in place etc .

Number: 13.5.1 IUse Category: REFERENCE Major Rev: 031 Minor Rev: 001 Trtle: Local, Protected Area or Site Evacuation Page: 9 of 20 4.2 Localized Evacuation Emergency Director Actions (Shift Manager, TSC Manager or EOF Manager) 4.2.1 For localized evacuations, determine if any areas of the plant should be avoided during the evacuation.

a. Define the area for a localized evacuation. Area should have well-defined boundaries (e.g., Radwaste Building, 422' Reactor Building, 501' Turbine Building, etc.).
b. In conjunction with RPM and the On-Shift Security Supervisor/ EOF Security Manager, evaluate the need to relocate security staff from BREs or other security positions.
c. Choose an Assembly Area for localized evacuations. This will normally be the onsite Yakima Building Conference Room, other areas may be specifiecL,.

  • d. Make a public address system announcement following the guidance provided on Form 26048, Public Address Emergency Message For Localized Evacuation. During Hostile Action situations in the PA or OCA, consult the On- -

Shift Security Supervisor prior to making PA announcements addressing evacuation details.

4.3 Protected Area Evacuation

.* f Emergency Director Actions {Shift Manager, TSC Manager or EOF Manager) 4.3.1 Determine appropriate Assembly Area for evacuees. The following guidance can be used to determine Assembly Area:

  • The Offsite Assembly Area (ENOC) may be used.
  • An onsite area outside the Protected Area may be used if only the concern is the Protected Area, monitoring is not required and evacuees are expected to return to normal work areas in a short period of time. Use the Deschutes Building, or Refer to Attachment 7.3, Triage and Staging Areas, for locations of pre-designated emergency staging and triage areas.
  • Personnel may be released to go home if monitoring is not required, security does not wish to retain personnel and you do not expect them to be returned to normal duties soon .

Local, Protected Area or Site Evacuation Page: 1O of 20 4.3.2 Direct protective actions for personnel based on recommendations from the On-Shift Security Supervisor or EOF Security Manager.

a. Modify announcements as necessary to obtain desired results.
b. Use fonn 26050, Public Address Emergency Message For Protected Area Evacuation to make appropriate announcements. During Hostile Action situations in the PA or OCA, consult the On-Shift Security Supervisor prior to making PA announcements addressing evacuation details.
c. IF announcement is made from the Control Room, THEN use the PA system override switch for announcements. Return the switch to the nonnal position when done.
d. Consider the need to relocate security personnel from BREs or other security positions. Coordinate actions with Security Management. This should not be part of the plant wide announcement 4.4 Site Evacuation Emergency Director Actions (Shift Manager, TSC Manager or EOF Manager) ~-:
  • 4.4.1 Confer with Shift HP Technician, the RPM or the REM to detennine any special instructions and/or preparations for evacuation, such as:
  • Is additional HP coverage needed for individuals existing the Radiologically Controlled Area (RCA) or the portal monitors at the Protected Area Access Point (PAAP)
  • Will radiological monitoring be required at the Offsite Assembly Area
  • Does the route to the Assembly Area need to be adjusted to avoid exposure
  • Special instructions to evacuees (keep windows closed, tum off A/C, etc)
  • No eating drinking or smoking while in route
  • Other actions which would limit exposure to dangers
  • Consider the need to relocate security personnel from BREs or other security positions. Coordinate actions with the On-Shift Security Supervisor. This should not be part of the plant wide announcement.
  • Coordinate with security or the ICP to detennine the best route of travel outside the PA

Local, Protected Area or Site Evacuation Page: 11 of 20 4.4.2 Confer with the On-Shift Security Supervisor to determine any other special measures that should be taken by evacuees. Special measures to consider:

  • Areas to avoid due to Security Issues
  • Recommended Routes to expedite evacuation.

Coordinate with DOE-RL if evacuation route requires access beyond the Wye Barricade (e.g. uses Route 11A).

4.4.3 IF radiological conditions, a security event or other unforeseen conditions prevents or alters implementation of the preplanned evacuation plans, THEN determine appropriate actions and revise the public address announcements accordingly.

NOTE: Relocation of security staff should not be included in the following public address announcement 4.4.4 Site Evacuation Routes Several evacuation routes are available to site personnel.

NOTE: Route 4 South will be referred to as Route 4 .

Route 1O South will be referred to as Route 10.

Route 2 South will be referred to as Route 2.

  • Route 4: This road leads north and south from the site.

It is the main route from the plant, south to Richland. ......

  • Route 1O: Accessed via Route 40 (FFTF access road) or Route~4 prior to the DOE Wye Barricade. It intersects with Highway 240'!"
  • Route 11A (DOE): Accessed via Route 2 or Route 4 at the DOE Wye Barricade. This allows evacuation to Yakima or Richland 4.4.5 Perform steps on form 26051, Public Address Emergency Message For Site Evacuation, to direct evacuation of the Site.   • IF the EOF Manager is the Emergency Director, THEN coordinate with the TSC Manager to make PA announcements.

4.4.6 Confirm with the On-Shift Security Supervisor or EOF Security Manager to insure actions for evacuation of the Industrial Development Complex (IDC) are being implemented when a Site Evacuation has been directed. {Ref 5.4}

4.4.7 IF evacuees are being directed to the Offsite Assembly Area, THEN direct the On-Shift Security Supervisor or the EOF Security Manager to contact the Benton County Emergency Operations Center (EOC) to request Local Law Enforcement Agency (LLEA) support for traffic and crowd control at the Offsite Assembly Area.

a. IF the EOF Security Manager is not available, THEN direct the On-Shift Security Supervisor to contact the Benton County EOC for LLEA support at the Offsite Assembly Area.  !'I.
b. IF the JIC is activated and the EOF Security Manager is NOT present,**

1) Initiate an lndusbial Development complex public address announcement by dialing 761 and reading the following message, or other announcement provided by the REM:

NOTE: Include alternate evacuation route instructions, if required.

This is an emergency announcement. Personnel in the exclusion area and the industrial development complex are to immediately evacuate the site and report to the assembly area at the east side of 3000 George Washington Way."

Repeat the PA announcement.

B. Radio Paging System / Site Evacuation Siren

1) Using the Radio Paging System at the EOF or the sec, make a Public Address'*:.,

announcement. {Ref 5.4}

2) Verify that the date and time appears on display of the Whelen E-969 Encoder labeled Site Evacuation Siren. The correct day and time display is not required to activate the IDC PA or evacuation siren. If the correct day or time is not displayed, contact the EOF Telecom Manager for resolUtion.
3) Press the "C2'; key The message "Call Key 2* appears in the display and the display will becom_e'i brighter.
4) Press the "Send" key
  • A timer in the display will count down from 180 to Owhile the activation codes are sent and the VA2000 unit's alert.
  • Message "Call key complete" will appear.
5) Using the PA microphone on the Encoder, press the push-to-talk button and make the following announcement "This is an emergency announcement. Personnel in the exclusion area and the industrial development complex are to immediately evacuate the site and report to the assembly area at the east side of 3000 George Washington Way."

Repeat the PA announcement .

8) Press the "Send" key
  • The message "Ca// key complete"will appear in the display.
9) Press the "Reset key to return to the E-969 to the standby mode.

1O) After the public address announcement is complete, direct the sec Access Control Officer to activate the Site Evacuation Siren.

11) If the sec cannot activate the Site Evacuation Siren, perform the following:

a) Initiate the 3-Minute Siren Alert Tone - Site Evacuation Siren (Cross Ro~ds Siren)

.i b) Verify the date and time appears in the display of the Whelen E-"969 Encoder "

labeled Site Evacuation Siren .

c) Press the "C1" key

  • The message "Ca// Key 1" appears in the display and the display will become brighter.

NOTE: The Site Evacuation Siren will time out automatically after three minutes.~ :~

d) Press the "Send" key

  • A timer in the display will count down from 180 to Owhile the activation codes are sent and the siren alerts.

13) If the Security Manager or sec are unable to activate the siren:
  • NOTE:

Contact the On-Shift Security Supervisor and direct a Security officer be dispatched to activate the Site Evacuation Siren manually.

Contact the sec for the current IDA phone list, if necessary.

14) Direct the IDA to perform a sweep of the Industrial Development complex.
15) If the sec has not been notified, contact the Industrial Development Authority (IDA) and verify that the Industrial Development complex evacuation is being initiated per PPM 13.5.7.

a) If no one can be contacted at the Industrial Development complex (377-1124),

make two attempts to contact personnel listed in the Industrial Development Authority Call List.

16) Contact the On-Shift Security Supervisor to inform security officers at the roadblocks of offsite assembly area location if necessary.
17) Coordinate with the On-Shift Security Supervisor to dispatch an officer with a radio to the assembly area to maintain order at the designated assembly area, and to relay messages or directions to evacuees .

18) Provide telephone notification of the evacuation and the above instructions to the following locations. Refer to the Emergency Phone Directory for phone numbers:

- Security Training Facility/Firing Range

- Ashe Substation

19) Contact the sec and have them contact the BREs to use their cameras to verify there is no activity, personnel or vehicles at the HSSF facility or vicinity.
20) Keep the Emergency Director infonned on the status of the evacuation.

C. Correcting Day and Time on the Whelen E-961 Siren Encoder

  • If either the day or time displayed is incorrect, the encoder should be reprogrammed as follows:

NOTES: The programming mode may be exited at any time by pressing the "RESIT button. Any changes made up to that point will not be saved, i provided that the "PRE CHECK" button was not pushed

  • f; The "PRECHECK" button is equivalent to the "ENTER" key during Encoder
  • programming .

Pressing the "DOWN" arrow more that once will select menu options that should not be accessed by anyone except Telephone Technicians. Do not navigate to any other menu option except the "Clock" menu

1) In the Command Group keypad area, push the "Test & Misc" button. The Lcb,display will illuminate.
2) Press the "DOWN" arrow ONCE to scroll to the Program Encoder screen and press the "PRECHECK" button to display the word, "CLOCK." Press the "PRECHECK" button again to display "PROGRAM ENCODER" and "ENTER TIME, DAY##:##:##."
3) First, enter the day of the week using numeric equivalents.

1 = Sunday 5 = Thursday 2 = Monday 6 = Friday 3 = Tuesday 7 = Saturday 4 = Wednesday Select the appropriate number on the numeric keypad and the day of the week displays .
