D43.02 This letter provides Post Examination Materials for the NRC License Examination that was administered on July 13, 2020, through July 29, 2020, at South Texas Project (STP), in accordance with NU REG 1021, Operator Licensing Examination Standards for Power Reactors.
STP Nuclear Operating Company requests the NRC consider Attachment 3 to this letter as Exempt from Public Disclosure. None of the exam materials are to go to the Public Document Room (ADAMS) for at least two years.
There are no commitments in this letter.

This question is based upon knowledge of valves that receive a Phase A Containment Isolation Signal and which valves receive an SI signal. FV-0011 receives a Phase A Containment Isolation signal and MOV-0059 receives an SI signal. FV-0011 is located on CP-004 and has red-green indication there. MOV-0059 is located on CP-0002 and has red-green indication there. Training in LOT 201.06 for CVCS and LOT 201.12 for CCW cover how these valves operate.

This question is based upon knowledge of loss of the instrument air system to the MAB. The student was required to know that Charging Flow Control Valve FCV-205 fails open upon a loss of instrument air. It also required knowledge of what action must be taken upon a failure of FCV-205. The required procedural action is to place the "Alternate Charging Path with FCV-205 Inoperable" in service. The correct answer to the question states that the "alternate charging path" is to be placed in service. Training given to be able to evaluate this question is covered in LOT 201.06 for CVCS and LOT 505.01 training on Off-Normal Procedures.

This question is based on knowledge of entry conditions for Technical Specifications. The correct answer, RWST Level Channel LT-0931 is removed from service for maintenance, is the only one that is a direct entry into a Technical Specification. General training on TSs is given in LOT 503.01.

This question is based upon knowledge of the UFSAR requirements of the AFW system. The AFW system is required to provide 500 gpm within 1 minute of initiation and will cool down the RCS at 50 degrees F/hour. Training is given in LOT 202.28 for Auxiliary Feedwater.
No changes to the exam are warranted.
SRO #80 - 5 candidates chose distracter B, 1 candidate chose distracter "D." Correct answer is A.
This question is based upon the knowledge of Emergency Maintenance. The student was required to know that emergency maintenance is performed at the discretion of the Shift Manager. The question further asked if the system could be considered functional or must be declared inoperable. This question failed to note that per 0PGP03-ZA-0090, that the system must be declared inoperable but can be declared functional. The question was written that inoperable and functional were two distinct states which is not the case. (Steps 4.1.1.3, 4.1.1.4).
Two answers (A and B) WERE accepted for this question.
SRO #81 - 5 candidates chose distracter B, 1 candidate chose distracter "D." Correct answer is A.
This question is based upon knowledge of Technical Specifications Section 6.12.2 It required the knowledge of who has control of the key for Locked High Radiation Areas. The key is required to be controlled by Technical Specification Section 6.0, and may be maintained by either the Shift Manager or the RP Manager. General training on TSs is given in LOT 503.01.

This question required the students to know required actions from 0POP04-AE-0005, for a grid disturbance. With both North and South bus voltages reading 335KV, all offsite sources are INOPERABLE. This is a 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> action by Technical Specifications to restore at least one of the inoperable offsite circuits on each unit. Per 0POP04-AE-0005, while awaiting voltage restoration, the crew will transfer the 4160V ESF Buses to the UAT. General training on TSs is given in LOT 503.01. LOT 505.01 gives the training on Off-Normal Procedures.

This question required the students to have knowledge of the pump inservice testing program. If a vibration reading on a safety related pump falls into the ALERT range, this does not require the pump be made inoperable, but does require the frequency of testing to be doubled per 0PGP03-ZE-0022. The pump is not OPERABLE but NONCONFORMING, because this designation is used when a component has improper design, testing, construction, or modification. This procedure is covered during OJT Training for the LOT program and in LOT 802, SRO Administrative Procedures Training.

This question asked to identify the condition that would result in a LIGHT BLUE indication for a radiation monitor. The keyed answer was "loss of sample flow." This condition is actually indicated by DARK BLUE. This is indicated in 0POP04-RA-0001, Addendum 24, Page 1 of 27 (shown on the next page). Since DARK BLUE indicates a monitor loss of sample flow and LIGHT BLUE does not correspond to any of the choices on the question, STP recommends that this question be removed from the exam.
After discussion with the Chief Examiner, this question was removed from the exam and then the exam was regraded.
Note: This question and the next 2 questions contained errors that were not caught during the development, validation, and review process. Condition Report 20-8347 will address this issue.

This question asked the student for the effects on RCP seal injection upon a loss of instrument air. With a loss of instrument air, HCV-0218 fails open, increasing seal injection flow to all 4 RCPs (Failure position is seen in 0POP04-IA-0001, Addendum 5, page 2, on the following pages). The student was also required to state where total seal injection flow could be read. The keyed answer was on the ICS screen which has all 4 individual seal injection flows and also total seal injection flow. On the control panels, each RCP has a digital recorder that displays seal injection flow and seal return flow (Pictures of these recorders are on the following pages). There is also a trend for these parameters right above the digital display. The student can note total seal injection flow on ICS, then also quickly determine that total seal injection flow has risen if all (or even one) recorder trends have changed.
This question placed the student in 0POP04-IA-0001, Loss of Instrument Air. In MODE 1, if a loss of instrument air occurred, several other abnormal operating procedures would be entered because of effects on individual systems such as CVCS and Feedwater. In this case, letdown and charging flow would be affected as would seal injection flow (Failure positions are seen in 0POP04-IA-0001, Addendum 5, page 2, shown below). The crew would respond with procedures to correct these issues. As part of the response for the effect on the CVCS system, the crew would need to know how to determine total charging pump flow which includes charging flow to the loops and seal injection flow. Total charging flow must be maintained less than a maximum of 300 gpm. (see 0POP09-AN-04M8, page 73. Total charging flow is operationally maintained at no more than 240 gpm, 200 gpm maximum charging flow and 40 gpm seal injection flow).

(See 0POP04-RP-0002, page 4, Addendum 2, Page 1, shown below) Total seal injection flow is also needed to ensure proper RCP seal package operation. Numerous procedures require the crew to maintain seal injection flow between 6 and 13 gpm per RCP. (See 0POP04-RP-0002, Page 3). Total seal injection flow is operationally relevant for several reasons, and the reactor operator must know different methods to obtain this value. Both the ICS and the individual digital recorders are important to the reactor operator's job and must be known. STP recommend accepting answers A and B on this question.
After discussion with the Chief Examiner, it was decided to accept answer A and B. The exam was then regraded.
