CLOSURE OF NRC INVESTIGATION NO. 4-1998-004 l
Dear Mr. Hutchinson:
This is to inform you of the closure of NRC investigation 4-1998-004. This investigation was conducted by the NRC's Office of Investigations (OI) to determine whether there was deliberate misconduct et Se part of two fire watch personnel who were formerly employed at Arkansas Nuclear One MNO) by Kidd Janitorial, a contractor to Entergy Operations, Inc. The investigation was completed March 31,1998, This investigation was initiated by 01 in January 1998 based on information that Entergy had terminated the employment of two fire watch personnel who were involved in an incident in which a fire watch log entry was found io have been inaccurate. Based on the NRC staff's review of the evidence developed by Ol, and the information compiled by Entergy with regard to
. this incident, the staff has concluded that there was no deliberate misconduct on the part of l
either fire watch. The evidence indicates that these individuals may have exercised poor judgement in making a log entry when there was uncertainty about its accuracy. In fact, the inaccurccy was discovered as a result of one of the individuals bringing the matter to the attention of his supervisor.
The NRC considers this matter closed and plans no further action No response to this letter is required.
