Subject:

Residual Heat Removal (RHR) Heat Exchanger Leak While the reactor was in a scheduled refueling outage, on April 3, 1976, at approximately 5:00 a.m., a RHR heat exchanger leak occurred which released 6,000 gallons of reactor water to the plant effluent via the heat exchanger cooling water system over a six minute period. Calcula- -

tions made by the licensee showed the total release to be 4.1 C1, mostly i

1131. The dilution water flow during the time of the release was 1,000,000 gal / min. Subsequent hourly samples taken down stream showed only background activity. ll l

The Technical Specification allowable release rate was exceeded for the l six minute duration of the release. The release was 40% of the total l Quarterly limit. j Region I (Philadelphia) is investigating the cause of the hsat exchanger failure and the details of the release.

Media interest is not expected. The State of Pennsylvania has been

, informed. A press release is not planned.
