The information presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

Subject:
" CONTROLLED" DROP OF UPPER REACTOR VESSEL ASSEMBLY The upper reactor vessel assembly slated for Palo Verde Unit No. 2 experienced a " controlled" drop during handling operations at Combustion Engineering (CE), Chattanooga, Tennessee.
The upper vessel assembly includes the upper shell, nozzle course, and vessel flange.
The event occurred when the brake for the crane, which was holding the assembly, slipped, allowing the assembly to drop onto a cart.
The. car.t was crushed.
No damage was disclosed by a visual examination of the assembly h+2 by CE.
CE intends to radiograph the assembly to further verify whether damage was incurred.
