LOSS OF 450 ONE-MICR0 CURIE AMERICIUM 241 F0ILS DURING SHIPMENT Region III (Chicago) was notified by Region IV via telephone on

March 27, 1978 that a package containing 10,000 americium 241 foil sources of 1.0 microcurie each and 2,100 radium 226 sources of 0.05 microcurie each was shipped from the licens6e's facility *in
damaged in shipment.

The President and RSO of Entronic were contacted by telephone on March 27, 1978.
They verified that a shipment as described above was made from Earth City, Missouri, on February 15, 1978, and the package arrived at 6;
Houston, Texas, in a damaged condition.
Eastern Airlines personnel removed the damaged package from the aircraft and notified Entronic, who picked up the damaged package on February 18, 1978.
The shipment was made on Eastern Flight No. 515 and the aircraft traveled from St. Louis, Missouri, to Houston, Texas, to Corpus Christi, Texas.
One of the vials that had fallen from the cardboard shipping container was recovered in the cargo area of the aircraft when Flight 515 arrived in Corpus Christi, Texas.
After making an inventory, Entronic's personnel determined that three vials containing a, total of.450 americium 241 sources of 1.0 microcurie each were missing.
Eastern, Airlines personnel will be contacted to determine that follow-up actions have been taken.
