Subject:

PERSONAL RADIATION EXPOSURE IN REACTOR CAVITY l

At 3:00 a.m. on May 2,1978, an unplanned radiation exposure occurred when a shift supervisor briefly entered the reactor cavity during a refueling outage to look for leakage from the refueling cavity. The individual did not carry a survey instrument during the entry, nor were adequate radiation surveys made beforehand. Initial evaluation by the licensee (based on the TLD worn by the individual) indicates a ,

dose of 2.8 rems. Radiation levels rates up to 2100 rems per' hour were sn measured in the cavity after the individual's exit. NRC inspectors are j currently investigating the circums,tances surrounding this event. t On September 19, 1976, IE Circular No. 76-03 was sent to all reactor -

licensees in an effort to prevent this type of incident (similar  ;

events were experienced at Point Beach, Zion, and Indian Point). t No press release has been made by the licensee nor Region III. The State of Wisconsin has been informed.

RIII (Chicago) inspectors were informed of the incident when they arrived onsite to conduct a routine inspection. The above reflects information as of 9:00 a.m. on May 4,1978.
