June 19, 1978 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE -- PN0-78-125 This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance.

Subject:
JET PUMP RISER CRACK Iowa Electric Light and Power Company notified Region III that a 3" long l
crack was found in a ten-inch recirculation system line.
The crack, which is spraying water, is in the horizontal run and appears to be in the Inconel transition piece between the pipe and the vessel nozzle.
l The reactor'had shutdown on June 17, 1978 due to an unrelated problem l
experienced during a surveillance test.
Primary system leakage was l
observed to be increasing before the shutdown.
g It is anticipated that an outage o,f at least two weeks will be required to complete the repairs.

1 Region III and the licensee intend to issue a press release.
The State of Iowa is being notified.
Region III (Chicago) received notification of the reactor shutdown and system leakage by telephone from the licensee at about 1:15 p.m. on June 17,1978. The location of the crack was identified on June 18, 1978. This PN reflects available information as of 8:00 a.m. on June 19,1978.
