l 1.0 OVERVIEW
1.1 Background
In August 1985, Eigen Engineering Inc. was contracted to perform a Verification and Validation Program on a Hope Creek SPDS which had been implemented by adding a L new set of CRT displays to the existing Control Room Information Display System (CRIDS).
Since the Hope Creek system was already installed and operational, no attempt was made to validate the basic design of the system. Instead, the V&V program concentrated on how well the final system configuration and performance met regulatory requirements.
The V&V Team prepared a program plan meeting the intent of NSAC/39. The Design Review Report (PSE-3210-87-001, Rev. O) was issued in February 1987.
Although the design of the system would, with some modifications, have been acceptable, the fact that the CRIDS was also performing several additional functions caused the overall SPDS performance to fall below acceptable levels. Thus, in March 1987, PSE&G decided to transfer the Hope Creek SPDS functions to a separate, stand-alone system.
This " stand-alone" system (Figure 1) consists of redundant Gould/SEL Computers interfaced with ten CPI remote multiplexer.
In October, 1987, Eigen Engineering, Inc. was contracted to perform the V&V program on the new Hope Creek SPDS.
In December, 1987, Eigen Engineering submitted a revised V&V Plan (Reference 1.3.12) detailing the work to be performed. As in the earlier V&V, a " performance verification" approach was used.

l V&V FINAL REPORT l 1.2 Summary of V&V Plan The V&V program plan addresses the concerns of an NRC audit performed August 27 and 28, 1985 (ref. 1.3.18).
The initial implementation of SPDS, upon which the audit was based, was installed on the plant control Room Integrated Display System (CRIDS). As a result of the audit, and performance concerns with the CRIDS, PSE&G decided to install a separate computer system for the SPDS. To address the V&V concerns documented by the audit, an "after-the-fact" V&V program - as documented in the letter referenced above - was performed on the new SPDS implementation.
The V&V program for the Hope Creek SPDS was performed according to the V&V plan, reference 1.3.12. The plan !
defines four basic ' activity areas:

Design Review Performance Validation Test Field Verification Test The plan discusses the method, and defines the required documentation for completing each activity area.
The following sections summarize the objectives, activities, and results of each of these areas.

1 V&V FINAL REPORT 2.0 REQUIREMENTS REVIEW 2.1 objectives The objective of the Requirements Review was to.
identify, and document the system requirements for Safety Parameter Display Systems based on NRC guidelines reference 1.3.1, and 1.3.2. These requirements became the basis for. subsequent reviews. .
The objective of the Requirements Review differs from that outlined in NSAC/39 (ref. 1.3.3). As stated in reference 1.3.18, section 3.1.2:
"EEI [Eigen Engineering, Inc.] intends to independently identify and list the system requirements and the sources of the requirements, as described in regulatory documents."
This deviation from the recommended NSAC/39 guidance is 4 the result of using an existing design, in which the '
requirements were established and documented prior to the V&V process.
2.2 Summary of Activities NRC documents NUREG 0737 Supplement 1, NUREG 0800, and NSAC/39 were reviewed to compile a list of system c requirements.
The basic requirements for an SPDS are provided in Supplement 1 to NUREG 0737 (ref. 1.3.1) which is "...a distillation of the basic requirements....from the broad range of guidance documents that the NRC has issued. ...
Specific requirements for the Hope Creek SPDS were i;
developed from NUREG 0800, Section 18.2. (ref. 1.3.2)
This document expands the basic requirements of NUREG 0737, Supplement 1 into specific guidelines to be used ;
by the NRC in review of SPDS implementation. l In addition, three requirements were identified in NSAC 39, Section 2.0; and three were identified based on the NRC SPDS V&V audit of August 1985. ,

3.0 DESIGN REVIEW j 3.1 Objectives There were two basic objectives to the design review.
The first objective of the design review was to verify -
that the system described in the SPDS design l documentation (References 1.3.4 thru 1.3.6) fully addressed the requirements defined during the requirements review.
The second objective was to review test procedures to ensure that design inputs were used, and system response was adequately tested.
The V&V team attempted not only to confirm the adequacy {
of the design, but alsa to confirm the adequacy of the design documentation in order to ensure an adequate configuration control mechanism.
3.2 Summary of Activities 3.2.1 Design Documentation Review The design review methodology employed by the V&V team for each requirement was to;
- 1. Identify the task (s) necessary to evaluate the system with respect to the requirement.
- 2. Summarize tne information evaluated.
- 3. Identify the specific references used in the evaluation.
- 4. State a conclusion regarding the adequacy *)
of the system with respect to the requirement.
This activity is documented in section 3.0 of the VGV Design Review Report (ref. 1.3.13).
The Hope Creek SPDS design philosophy, ac described in Reference 1.3.4, Section 2,0 is based on the concept that " plant safety can he assured for a wide range of events and sovere accidents, through adherence to the EOP's and maintaining PSE&G 10 NOV-1000-88-003 REV.1

The purpose of the SPDS, therefore, is to provide a focal point for the control room operators to obtain the information necessary to make the decisions required by the EOP(s).
The V&V team accepted this philosophy and thus based their review of the system's parameter set and display features on this principle. In addition, the design documents represent the current display design baseline for configuration control purposes. i 3.2.2 Test Procedure Review The test procedure reviews were performed subsequent, and independent of the design document review. .
Four separate test procedures were written:
Factory Acceptance Test (ref. 1.3.7)
Field Verification Test (ref. 1.3.9)
On-Site Validation Test (ref. 1.3.10)
Simulator Dynamic Performance Test (ref.
1.3.11)
The first test, factory acceptance, was developed and performed by NOVA Technology. The second and third tests, collectively refered to as
" reoperation tests," were developed and written by PSE&G. The final Simulator test was also developed by PSE&G.
The NOVA Technology factory acceptance test, Hope Creek SPDS Acceptance Test Procedure (ref. 1.3.7),
was reviewed and witnessed by the V&V team.
The preoperational tests (ref. 1.3.9 and 1.3.10) had been. subject to multiple levels of site review and approval. On that basis, the V&V review was conducted on an audit basis, i.e. only selected portions of the testing were subjected to detailed review. !
The Simulator Dynamic Performance Test was written and reviewed internally by PSE&G.

V&V FINAL REPURT 3.3 Summary of Results The results of the Design Review are documented in the V&V Design Review Report, reference 1.3.13.
The results of the Factory Acceptance Test review are documented in reference 1.3.14.
The results of the Preoperational Test Procedures review are documented in the V&V Preoperational Test Review Report, reference 1.3.15.
The Simulator dynamic performance test was not reviewed by the Eigen V&V team. The PSE&G performance test report, reference 1.3.17, addresses the concerns defined in the V&V plan.
3.4 Summary of Deficiencies Identified / Resolved 3.4.1 Design Document Review A total of 105 discrepancies were found in the design documentation. Additionally, there were 22 open items identified from review with the system requirements. These discrepancies are numbered 1-105, and 265-286, respectively, in Appendix A.
3.4.2 Test Procedure Review A total of 26 discrepancies were reported for the Hope Creek SPDS Acceptance Test Procedure, reference 1.3.7. These discrepancies are numbered 106-131.
A total of 10 discrepancies were reported for the Hope Creek Parameter Display System Preoperational Test Procedure For Multiplexed Digitial and Analog Inputs, ref. 1.3.9. These discrepancies are numbered 166-175.

'4.0 VALIDATION TEST 4.1' Objectives IAs: described in NSAC/39, system validation testing is performed to demonstrate that the integrated system meets the requirements.
4.2 Summary of Activities Validation testing for the Hope. Creek SPDS was performed in the following tests:
4.2.1 Factory Acceptance Testing.
This procedure (Reference 1.3.7) tested the functional' adequacy of major system components with respect-to system power-up,. terminal communications, system failover,1 system displays-and database verification.
4.2.2 Integrated System Testing The objective of this test (Reference 1.3.10) was '
to verify that-SPDS hardware and software design-and installation was in accordance with the design philosophy delineated in,the System Design Memoranda.
During installation testing (see 5.0 Field Verification Test) all unique point inputs from the system multiplexer through the Data Input Processor to the system displays were tested.
During integrated system testing input values were simulated into the Display Input Processor to test composed points, complex algorithms, data manipulation, displays, trends, and Man Machine Interface (MMI) features.
In certain cases, where factory acceptance testing had been satisfactorily performed and documentod, only a sanple test was performed during integrated System testing. For example, all point summary displays were tested during factory acceptance
~t testing. During integrated system tasting, therefore, only a sample of the point summary displays were tested.

EMP-009 " Operational Design Change Control" and the guidelines of Design Memoranda
,H-1-RJXX-CDM-0599 "HCGS SPDS software and Database Configuration control". A prerequisite of integrated system testing was to verify the software and database loaded onLthe' computer are in configuration with the Master SPDS software and database files.
Integrated' system testing'also included data links from the Radiation Monitoring System (RMS) and' Uninterruptable Power Supply (UPS).
4.2.3 Dynamic Performance Testing The objective of this test 1(ref. .1. 3.11) ' was to
. validate the effectiveness of the-SPDS by performing plant transient scenarios on the. Hope Creek simulator. These tests were rtin in accordance with the V&V Plan to show response to transient and accident sequences (see requirement, 3.1.9 in V&V Design Review, reference 1.3.13).
The Hope Creek simulator complies with NRC.
requirements as documented in a letter sent to the.
NRC December 14, 1989 (reference'1.3.19).
The Simulator Dynamic Performance Terting used three' scenarios,.and three operator crews. Each scenario was run twice, once with, once without SPDS. The following data was recorded for'each scenario:
- 1. The elapsed time required to enter the appropriate EOP.
- 2. The elapsed time required to exit the appropriate EOP.
' 3. The worst case value of the EOP entry parameter.
Following each scenario set, operators were asked for their input.

qualitative determination of performance since no j real significance can be associated with any i absolute measurements. {
l 4.3 Summary of Results The results of the System Validation Testing V&V review are included in References 1.3.14, 1.3.15, and 1.3.17.
4.4 Summary of Deficiencies Identified / Resolved 4.4.1 Factory Acceptance Testing Factory Acceptance Testing conducted using Document reference 1.3.7 resulted in 34 test exceptions. These exceptions are documented in discrepancy reports 132-165.
4.4.2 Integrated System Testing Testing conducted using Document reference,1.3.10 resulted in 26 test. exceptions. These exceptions are documented in discrepancy reports 239-264.
4.4.3 Dynamic Performance Testing The Simulator Dynamic performance Testing conducted using Document reference 1.3.11 produced no discrepancy, or exception reports due to its qualitative nature.
t Interviews with operators following the Simulator Dynamic performance test did not provide meaningful input due to the operators' limited experience with SPDS prior to performance of the test.

V&V FINAL REPDRT i 5.0 FIELD VERIFICATION TEST 5.1 Objectives The objective of the Field Verification Test was to ensure that all field inputs were properly installed.
5.2 Summary of Activities A test procedure, Hope Creek Parameter Display System Preoperational Test Procedure For Multiplexed Digitial and Analog Inputs, M13-STP-004-4EC-1076, Rev. 0 ,
(reference 1.3.9) was developed by PSE&G to verify all l field inputs.
The plan was reviewed by the V&V team - Eigen Engineering - as discussed in section 3.0 above and documented in reference 1.3.15.
Since much of the validation testing was performed during preoperational testing of the installed system, it was necessary to perform installation verification prior to system validation.
At Hope Creek, this was accomplished-in the following tests; Initial Checkout and Energization Testing (Reference 1.3.8, DCP's 2, 3 & 4)
This testing included cable continuity, card calibration, system diagnostics, and initial energization of equipment.
Installation Testing This test (Reference 1.3.9) verified the correct design and installation of the 10 SPDS multiplexer as delineated in the System Design Memoranda.
The scope of this procedure was to test each field input to the system multiplexer and verify that field data could be transported ;
thru the system and stored in (or accessed from) the data input processor, i

V&V. FINAL REPURT In addition, digital points were also tested for point specific failures, tag sets, and contact normal status. Analog points were tested for point accuracy, point specific failures, process value units, and alarm values. Each point was also tested for point overlap to ensure.the SPDS indicated value is in agreement with actual plant parameters.
No V&V activities were performed during initial system checkout and energization.
Due to the testing sequence,'any unresolved problems would be identified and corrected during subsequent tests.
5.3 Summary of Results The results of System Field Verification V&V review are contained in Reference 1.3.15.
5.4 Summary of_ Deficiencies Identified / Resolved Performance of the Hope Creek Parameter Display System Preoperational Test Procedure For Multiplexed Digitial and Analog Inputs., reference 1.3.9, resulted in 20 test exceptions. These exceptions are documented in discrepancy reports 176-195.

SUMMARY
AN) CONCLUSIONS 6.1 Summary 1 During the course of the V&V program, the V&V team identified 286 discrepancies (see Appendix A). When Eigen Engineering submitted their final V&V report (ref. 1.3.16), they identified nine open discrepancies. l l
PSE&G has resolved all of these open items. Three !
items which pertained to documentation that had not j been received, have been closed (references 1.3.20, 1.3.21, and 1.3.23). Two items which remained open awaiting completion of the simulator dynamic testing have been closed.
The remaining four items required testing during the mid-cycle outage in February, 1989. These tests have been identified, performed, and documented in reference 1.3.8.
While resolutions to all discrepancies have been identified, final closure requires final revisions of documentation. This work is currently in progress.
i 6.2 Conclusions The following conclusions are those of Eigen Engineering Incorporated. With the exception of paragraph numbering, and comments within square brackets, the conclusions are taken, in whole, from the
" Summary and Conclusions" section of the Final V&V report prepared by Eigen Engineering (ref. 1.3.16).
The objectives of the V&V program are to provide reasonable assurance that the four design requirements stated in Section IV of NUREG 0800 are satisfied. With respect to these requirements, the V&V team feels that; A. The variables displayed by the SPDS are sufficient i to provide the minimum information required to assess !
the critical plant safety functions.
l B. The SPDS is suitably isolated from electrical and electronic interference with equipment and sensors that are used in safety systems. !

V&V FINAL REPORT 6.2.1 Display Content and Format The content and format of the Hope Creek SPDS displays adequately meet all applicable system requirements. In fact, coordination between specific SPDS displays and specific EOP's provides a.very effective way of meeting the information needs-of the plant operators during emergency operating conditions.
6.2.2 Data Scan Intervals The instrument scan rates and data throughput times specified in the design documents are adequate.
6.2.3 Scale Optim.s ,lon Scale optimization of the SPDS is considered adequate. During an NRC audit of the previous ,
SPDS, trend graph time scales were considered inadequate for transient analysis. In the opinion of the V&V team, these scales are adequate for the stated SPDS purpose of trend monitoring in support of EOP ' s.
6.2.4 Data Validation The EPRI data validation methodology utilized by the SPDS is considered adequate.
6.2.5 SPDS Failure In the opinion of the V&V team, SPDS failure recognition, operator response to SPDS failure, and determination of SPDS operability, are adequately addressed by the system design and operator training.
6.2.6 Electrical Isolation In the opinion of the V&V team, the use of optical isolators provides adequate Class lE isolation.
6.2.7 Configuration Control In the opinion of the V&V team, the configuration control program identified in GM8-EMP-009 PSE&G 20 NOV-1000-88-003 REV.1

As described in other sections of this test, failure of an SPDS computer,. MUX communication link.and/or UPS communication link will give an "SPDS SYSTEM TROUBLE" annunciator. No alarms are mentioned in this section resulting from an RMS computer failure.
Disposition: Failure of any syste, status display communication link causes the top MUX block to turn white causing.the annunciator alarm. The test verified that the top MUX health indicator changing from green to an alarm condition causes the annunciator to actuate.
