NRC INSPECTION REPORT NO. 50-456/98005(DRP); 50-457/98005(DRP)

AND NOTICE OF VIOLATION FOR BRAIDWOOD

' Dear Mr. Kingsley:

On April 20,1998, the NRC completed an inspection at your Braidwood Units 1 and 2 reactor

facilities. The enclosed report presents the results of that inspection. During the 6-week period

covered by this inspection, your conduct of activities was generally characterized by safety-

conscious operations and sound engineering and maintenance practices.

We are concemed, however, about three violations involving the design or changes to the design

of the spent fuel pool. The first violation involved the failure to follow the temporary alteration

procedure when a change was made in the method to control the height of the spent fuel pool

skimmer suction strainer. The second violation pertained to the design and maintenance of the

spent fuel pool skimmer system which did not ensure that Technical Specification requirements

to prevent inadvertent drain-down below an elevation of 423 feet were met. The improper

. maintenance of the spent fuel pool skimmer configuration was identified by your staff, however,

- we are issuing this violation because the maintenance concem was identified as a follow-up to

the inspectors concem with the improper design. Finally, the introduction of a temporary filtration

system into the spent fuel pool in 1995 was initially determined not to be a temporary alteration,

then in September 1997, after re-evaluation it was considered a temporary alteration by a

Nuclear Fuel Services engineer; however, a problem identification form was not initiated to

assure proper corrective actions were taken.

Two non-cited violations are discussed in this report. The first violation involved the failure of

operators to follow the procedure to change the spent fuel pool skimmer system filter. This

,

resulted in the improper installation of the filter and loss of about 3 inches of fuel pool water level

before operators secured the system. The second non-cited violation involved a Technical

l

Specification violation of the containment spray additive flow rate caused by an inadequate

l

surveillance test procedure.

The first three violations above are cited in the enclosed Notice of Violation (Notice), and the

'

circumstances surrounding the violations are described in detail in the enclosed report. Please

' note that you are required to respond to this letter and should follow the instructions specified in

the enclosed Notice when preparing your response. Your response should not only detail the

actions to be taken for the specific issues identified, but should also be broadly focused on what
