Document Control Desk June 5, 1989 Attachment Duke Power Company McGuire Nuclear Station Unit 2 Vibration and Loose Parts Monitor Special Report On April 25, 1989 channels 4, and 6 of the Unit 2 Vibration and Loose Parts Monitor were observed. to have a low noise level. The likely cause of channel failure is due to bad or daaaged cable to the sensors on each affected channel-It was not possible to determine the definite cause of of the failure due to the number of possibilities, (e.g., bad cable, failed accelerometer, failed pre-amp, etc..) and the location of these devices prohibited inspection during unit operation.

The Vibration and Loose Parts Monitor (VLPM) were manufactured by Rockwell International. The VLPM cables have had a history of being susceptible to damage causing system failure. Exact replacements for the cables used are no longer available fron Rockwell International. An acceptable substitute has been offered .

by Rockwell and approved by Duke Design Engineering, and have been ordered by McGuire personnel.

The failed cables are inside the containment building and are not accessible during unit operation. .The VLPM will be repaired during the next refueling outage after the new replacements are received .

At this time, no compensatory measures are required or planned.

This item is not reportahle to the Nuclear Plant Reliability Data System (NPRDS).
