Gentlemen:

Subject: NRC Inspection Report No. 50-333/89-13

A special radiological controls inspection was conducted by Mr. R. L. Nimitz

of this office during the period of June 13-15, 1989. 'Ihe purpose of this

inspection was to review the circumstances, evaluations and corrective actions

associated with the unplanned exposure of personnel on June 12, 1989. The

findings of the inspection were dimed with you and members of your staff on

June 15, 1989. Areas reviewed during the inspection are di m m W in the

enclosed NRC Inspection Report.

One apparent violation was identified and is described in the enclosed Notice

of Violation. The violation involved inadequate radiological surveys for Spent

Fuel Pool work activities. You are required to respond to the Notice ard should

follow the instructions contained therein.

Our review fourd that effective action was taken to retrieve the object that was

the source of radiation for the unplanned perc h exposures. We also found

that effective action was taken to preclude exposure from similar sources which

may have been present at the time. The dose ve~-uts performed for the

personnel involved were acceptable. However, the unplanned exposures suggest

lack of effective oversig}t of radiologically significant Spent Fuel Fool work

activities. In your reply. please provide details as to the actions which have

been or will be taken to ensure perronnel workirg in and around the Spent Fuel

Fool will be protected frun unplanned exposures during handlire of highly

radioactive componerits and equipment contained in or removed frun the Spent Fuel

Pool.
