FEMA FINAL REPORT ON THE FARLEY EXERCISE OF NOVEMBER 2, 1988 Enclosed is a copy of the report by the Federal Emergency Management Agency (FEMA) on the Farley Nuclear Plant emergency response exercise randucted on November 2, 1988. As indicated in the report, FEMA identifief one deficiency, which was promptly corrected. Also, 6 arens requiring corrective action were identified by FEMA during the exercise, as well as an additional 29 areas  ! recommended for improvement. I We encourage you to assist the appropriate organizations in resolving the weaknesses identified by FEMA. SUMMARY

l* ' The full participation joint exercise of the off-site plans and j preparedness for the Farley Nuclear Power Plant was conducted on ' i November 2, 1988. The exercise was evaluated by a team ,of 23 Federal evaluators representing six Federal agencies. 'The  : evaluation was based on " Criteria for Preparation and Evaluation i of Radiological Emergency Response Plans and Preparedness in l Support of Nuclear Power Plants", NUREG-0654/ FEMA-REP-1, Rev. 1. The Farley Nuclear Power Plant, operated by the Alabama Power I Company, is located in Houston County, approximately seventeen miles east of the City of Dothan, Alabama, on the Chattahoochen River. The station has two pressurized water reactors, each  ; l capable of producing 829 megawatts of electricity. 1 The State of Alabama activated its State Emergency Operations { l Center (SEOC) and Radiological Control Agency offices in j I Montgomery, and the State Central EOC in Dothan. The State of ' Gs rgia activated its Forward ECO in Blakely, Georgia. Houston i County, Alabama and Early County, Georgia, fully activated their j l EOCs and their emergency response organizations.  ! l The exercise included the following major participants: I Alabama Emergency Management Agency ' Alabama Radiation Control Agency Georgia Emergency Management Agency l Dothan/ Houston County Emergency Management Agency Blakely/Early County Emergency Management Alabama Power Company One " Deficiency", sIX " Areas Requiring Corrective Actions", and I several " Areas Recommended for Improvement" were identified during the exercise. The " Deficiency" was identified for Houston I county as a result of Alert and Notification siren failures. A successful remedial drill to correct this deficiency was conducted on November 9, 198G. The deficiency, requirements and I recommendations are listed in sections III and IV of this report. l

State of Alabama I State Emercency Ooerations Center (SEOC) - Montaomery The State EOC is located in the Alabama Emergency Management  ! Agency (AEMA) offices in Montgomery. The space, while small, was  ! well utilized and sufficient to support emergency operations. j The EOC was notified of the Unusual Event at 8:29 a.m. by Alabama Radiological Health. Houston County was notified by AEMA at 8:30 } a a.m. When notified of an Alert at 9:04 a.m., AEMA called Houston l l County immediately and then began calling other state agencies on the call list. At 9:20 a.m. this task was completed. Although , not required to be present until a Site Area Emergency { l declaration, many state agency representatives came to the EOC l after they were notified of an Alert. Agencies represented i included the National Guard and the Departments of Human ' i Resources, Public Health, Public Safety, Highway, Conservation l and Natural Resources, and Agriculture and Industries. Security for the EOC was provided by the Capitol Police. Notification of a Site Area Emergency was received at 10:07 a.m. General Emergency notification was received at 12:15 p.m. After receiving the health order to establish access control i I within the 2-mile radius and to notify the public, the Operations  ! Officer coordinated with the State of Georgia and Houston County l the decision to activate the Public Notification System at 10:40 a.m. [ The Central Emergency Operations Center in Dothan assumed control at 10:40 a.m., and the State EOC in Montgomery stood by in a support role. The Alert'was received at 8:59 a.m., and the Site Area i Emergency was received at 9:59 a.m. Standard proc 6dures were followed, and the appropriate notification was nade at each classification level. l Staff was kept well informed through the use of status boards and periodic briefings. Although a map of the EPZ has provided, it was not utilized to plot the plume direction. I At 11:04 a.m. Radiological Health at the CEOC in Dothan assumed control. State Radiological Monitorina Assessment Center (SRMAC) - Dothan Staffing was adequate and personnel were competent. Checkliets

The space for SRMAC operations was limited. More space could be allocated to the SRMAC when it moves to the new county building. Maps and displays were adequate, but could be upgraded to enhance operations. Communications problems were encountered during the exercise. The Emergency Notification Network (ENN), instciled and maintained by Alabama Power Company, functie.99d sporadically. Information flow between the utility and-SRMAC was net always prompt. Meteorological updates, which the utility was supposed to provide to the SRMAC every 15 minutes, were late, and information concerning the presence of iodine was received 40 minutes after the release. SRMAC team members dealt with these problems effectively. The County and State Radiological Health response effort was satisfactory and appropriate. This co-location of staffs implemented an " Area Recommended for Improvement" identified during the 1986 exercise.

PIO representatives were present at the Media Center from the States of Alabana and Georgia, Early County, Georgia, and the Alabama Power Company. Coordination of news releases was excellent, as was general coordination and rapport between staffs. Communications, both internal and external, were 1 4 adequate. j i Two media briefings were conducted. Actual and mock media were present for the first briefing; only rock. media cera present for the second. Media representatives posed a number of difficult questions which were adequately addressed by the six spokespersons preseht. However, spokespersons could have been more assertive and precise in their briefing. For example, while discussing the failure of the sirens, the spokesperson did not give the number of siren failures or describe the percentage cf the public alert and notification system the sirens represented. Adequate displays were available and utilized by the ' i spokespersons to explain plant status and to point out the EPZ sectors affected by the implacentation of protective actions.- Prior to each media briefing a coordination meeting of the six spokespersons was held to ensure that there would be no conflicts or misrepresentations during the briefings. Rumor control was adequately handled by a staff of two. Radiological Field Monitorina Team RC-12 J Team RC-12 was adequately briefed by the state representative on , the status of the plant prior to departure for the field. The i Standard Operating Procedures (SOP) manual contained an equipment  ! list, appropriate data forms, and procedures for monitoring and sampling. All equipment had been properly calibrated.  ! l The team had a fixed VHF/UHF 8-band radio in their vehicle and were able to communicate effectively throughout the exercise. They also served as a relay between the Houston County EOC and '1 Team RC-14. The team was kept up-to-date on the plant status and i meteorological conditions. The team demonstrated good health physics practices from the l checkout of instruments to the taking of radiological measurements en route and at designated locations throughout the exercise. They had an excellent map system and readily found all locations. They recorded all necessary data and appropriately I controlled their accumulated exposure. Radiological Field Monitorina Team RC-14 Field team RC-14 arrived at the Houston County Health Department deployment point at approximately 10:30 a.m. The team had all the equipment identified in the plan and SOP. The team was sent to the Houston County Rescue Unit depot near Cowarts at 11:00 a.m. and was to relieve Houston County acaitoring personnel by 11:30 a.m. At 11:47 a.m., the team was deployed to a point within one mile of the plant to collect an air (iodine) sample. En route, the team received an-instruction to rendezvous with Team RC-12 at a point approximately four miles  ! i from the plant to obtain a milk sample already collected by RC-12 and then to proceed with both samples to the mobile lab at the Houston County Health Center. Just before reaching the sampling point, Team RC-14 lost radio contact with the Team Controller in the EOC. A vs.ry broken-up r j l segment of the " General Emergency" declaration message was heard over the GE Perta-Mobile II radio at 12:12 p.m., but the team ' could not verify the message because they were out of range and there was no back-up radio in the vehicle. In order to re-establish radio contact, the team drove northward to a high point on the road where they were able to contact the team controller via relay through RC-12 and receive the full text of the General Emergency message. At 11:00 a.m. on November 9, 1988, the three sirens in the Farley Prompt Notification System were activated from the Sheriff's dispatch room. All siren: activated and functioned properly. This demonstration corrects the deficiency identified during the November 2, 1988 exercise, i Houston County Forward Command Post (FCP) The Houston County Forward Command Post is located in the Enon Rescue Building on Enon Road ten miles from the Farley Nuclear Power Plant. The facility consisted of a large operations room, office space, e radio room, kitchen, rest rooms with shower, and two vehicle bays. This facility was adequately equipped but could utilize a tone-alert radio in order to monitor emergency l broadcasts to the public. This facility was used as an assembly point for emergency workers entering and leaving the ten-mile EPZ in Alabama. Tna FCP was staffed by both paid and volunteer workers, all of whom were well trained and dedicated. Workers reporting to the l FCP were briefed on work assignments and issued dosimeters before I going into the field. On completion of their assignment, they returned to the FCP for debriefing and monitoring. l The State radio net coverage in the ten mile EPZ was incomplete. The correction of this inadequate coverage is planned. (F .1. d. ) l Emergency worker exposure control was adequately demonstrated. Each worker sas provided a wallet-size card listing dosage limits for emergency workers. This card was developed by the Radiological Health Branch of the Alabama Department of Health. Several emergency workers and their vehicles were monitored for contamination using a trigger level for decontamination of 0.1 mR above background. Radiation levels which would require decontamination were not found, and an actual decontamination demonstration was not conducted. The decontamination personnel were familiar with required procedures and " talked" the evaluator through the process. The Forward Command Post provided a vital link in communications as well as a roomy, comfortable assembly point for emergency workers assigned duties in the ten-mile EPZ. Superior Items: l 1. Eight volunteers from the Retired Senior Volunteers Program were processed through monitoring and registration. The level of staffing was adequate to support the l Center's activity for this exercise. Evacuees we'w registered prior to being monitored which was not i in accordanca wizh the County Plan and procedures. This happened I because the evacuees arrived before the monitors. Participants were cognizant of the correct procedures; however, the lack of pre-planning, evidenced in the need to rearrange the registration l l desks and the " flow" of evacuees would have caused problems in a real emergency. (J.12.) Effective procedures for insuring that all evacuees had been monitored prior to being registered were not in place. Although monitors had colored stickers to affix to evacuses, the registrars were unfamiliar with their significance. (0.4.) Radiological monitors and the County Health Nurse were under the impression that evacuees being offered KI would be referred to the County Health Nurse prior to registration, which is not in accordance with Department of Human Resources SOPS. distribution of KI was not authorized, the correct procedures for its issuance need to be understood by all reception center workers. (0.4.) 1 State of Georcia Forward Emercency Ocerations Center (FEOC) The Forward Emergency Operations Center at Blakely, Georgia, was staffed and equipped by Georgia Emergency Management Agency personnel at 8:35 a.m. The Governor's Reprtuentative officially activated the FEOC at 9:10 a.m. Communications were immediately established with appropriate off-site response organizations and confirmed by call list.

The FEOC command clearly demonstrated the ability to make decisions and coordinate emergency _ activities despite the limited impact of the scenario on Georgia and the constraints of the lack of the Department of Natural Resources participation due to a commitment to an actual radiological event. The recently-completed facility, housing both the FEOC and the Early County EOC, was well organized and equipped with appropriate maps and status boards. These displays were kept current and fully utilized. Periodic briefings were conducted with the Early County EOC. Command and coordination was excellent. While the only protective action required was the restriction of access into the two-mile EPZ, this decision was coordinated between the EOCs. Communication systems were successfully demonstrated during the exercise. However, the tone-alert radio signal was not received ct the FEOC, and the radio inside'the facility did not receive the public instructions. While the FEOC is beyond the ten-mile EP2, it would be beneficial for the FEOC staff to monitor public notification and instructions. The FEOC staff demonstrated the knowledge to make appropriate decisions and supervise the use of KI through the simulated deployment and use of monitoring teams. Suoerior Items:

Emeraency ODerations Center (EOC) - Blakely, Georcia ' l The Early County EOC's entire staff displayed an enthusiasm and  ! cooperative spirit that would well serve Early County, Georgia. A real concern for safety was evident throughout the exercise. The new EOC facility is spacious, well furnished, and proved to be an excellent EOC. The EOC staff utilized numerous professionally prepared maps and status boards which displayed the current ECL, weather data, protective actions, and traffic control points. 1 Early County had numerous modes of communication available, including six commercial telephone lines, five dedicated telephone lines, the ENN system, police / fire radios in the Sheriff's dispatch center, and facsimile machines. 'All 18

communications systems were demonstrated. However, messages on the ENN system, which were broken up, were the primary source of information from the plant. In addition, the tone-alert radio did net function properly, which prevented Early County from monitoring the radio to verify that the tone alert was successfully activated and appropriate messages broadcast. Activation and staffing of the Early County, Georgia EOC was prompt. An up-to-date call list was used to notify the staff to stand by during the Alert ECL. The Alert call list also included back-up names and addresses for use in extended operations. The EOC was fully activated at Site Area Emergency and operational at 12:14 p.m. A Public Affairs Officer was dispatched to the Joint Media Center (JMC) at 12:05 p.m., and Radiological Monitors were dispatched (simulated) to the field at 12:05 p.m. The Emergency Management Agency Director effectively managed operations. He coordinated decisions with the Early County Commission Chairman, the County Administrator, and other appropriate staff. Periodic briefings and situation reports were conducted by the State of Georgia (GEMA) and the Early County staff. Internal messages were handled according to the plan. Copies were distributed to appropriate staff for action. i The Department of Family and Children Services (DFACS) is responsible for the notification and evacuation of individuals ' with special needs. There was a current list of people with l special needs as well as resource availability. l l Early County utilized prescripted messages to notify the public l and coordinated their release with GEMA, AEMA, Houston County, I and the utility. In the interest of brevity each objective will not be repeated in this basic letter. Attached in tabular forn is the detailed list of the 36 objectives from GM EX-3. The format reflects in columns the year FEMA records indicate that the objectives was accomplished; and a "yes" is shown in the Alabama and/or Houston County columns if the objective will be included in the November 2 exercise at Farley NPP. Your attention is invited to objectives 18, 19, and 20. These objectives are scenario dependent. We intend to write the scenario based on the plume path proceeding easterly into Early County Georgia. However, we will identify a specific area in the Alabama 10 mile EPZ to demonstrate these three objectives. This problem-play will be a scenario within the scenario for a specific location and the actions necessary to cover the remainder of the EPZ will be simulated. We have a particular concern with objectives 21, 23, 24, and 25. We will demonstrate that there are vehicles, i equipment and personnel to transport contaminated and injured further we will show that there are medical personnel; 1

l facilities, eculpment end personnel to handle contaminated, injured or exposed individuals; also ne will demonstrate decontamination facilities, equipment, supplies and personnel. However, the State Radiological Control Agency has not been successful in obtaining guidance from FEMA on a definition of " adequacy" and approved procedures. It does not appear that sufficient thought has been gi/en to the difference between radiation contamination associated with nuclear weapons versus the contamination tnat may occur from a nuclear power plant accident. Therefore, we feel it incumbent on FEMA to provide recommended procedures with scientifically sound instrumentation to support the procedures before we can demonstrate " adequacy" of these objectives. If this issue cannot be resolved for this scheduled exercise or if your headquarters does not consider this the appropriate vehicle to resolve an issue of this magnitude, then we would ask that these four objectives be deferred until proper guidance is given by FEMA. ncerely, N

I believe the approach you have specified in your 15 August letter is j reasonable and appropriate given the existing circumstances. I noted, i however, that you did not make reference to radiation response personnel in our Department of Human Resources (DHR) Radiation Health Section. As I indicated in my letter of 5 August 1988, these personnel are also committed to the RS: incident.
