. Individual Medical Transport In Lieu of Required Medical Drill Portion of 1989 Annual Emergency Exercise Gentlemen:
an employee fell ' from a On. April.6, 1989,1 an Unusual Event was declared when ladder in the reactor building and was transported to Charlotte Memorial Hospital as,potentially' contaminated.
At the time Mr. A.L. Cunningham was conducting an -
' unannounced ir spection 'in the area of emergency preparedness.
Mr. Cunningham accompanied D,Ae Power personnel to the hospital and observed all related activities involved in the transport of the injured individual.
Details of the event are described in the attachment.
Therefore, based on the f act. that Mr. Cunningham observed the preparation and transport of the contaminated injured individual, we request that McGuire be given credit for this event in lieu of performing a contaminated injured individual medical drill during the McGuire Annual Emergency Exercise scheduled for October
-1989.
We also request that you provide your response to us by August 1, 1989 in order for us to finalize our emergency drill plans.

' U.S. Nuclear Regulatory Commission Document Control Desk' May.31, 1989 Page 1 ATTACHMENT Transportation of Contaminated Injured Individual Description of Event On April 6, 1989 at approximately 10:05 AM while performing a routine post-trip walk down inspection of the Unit 2 Reactor Building, the injured person (male Duke ea;ployee) fell-while descending a ladder.
The injured person had. fallen approximately four (4) feet landing on his back. The accident was reported to the Control Room and the McGuire Medical Emergency Response Team (MERT) was dispatched to the area at approximately 10:10 AM.
Af ter donning protective clothing, the MERT team entered the Reactor Building and
'found the injured person lying on the floor complaining of severe lower back pain.
The injured person's vital signs were checked, and he was placed in full spinal immobilization and transferred to a patient transport basket.
An' ambulance (MEDIC 10) was requested at 10:32 AM which arrived at 10:47 AM.
The injured person was transported to the Auxiliary Building First Aid Room where Health Physics -(HP) personnel cut and removed his protective clothing, and performed a radiological survey for contaminat',n on the anterior portion of his body. Due to the symptoms and suspected injury (spinal), HERT personnel advised against rolling the injured person onto his s+.omach or side to perform a contamination survey on the posterior portion of b b body.
At 11:20 AM, the Notification of Unusual Event (RP/0/A/5700/01) and NRC Immediate Notification (RP/0/A/5700/10) procedures were implemented because the injured person could not be demonstrated to be free of contamination because of his injuries.
The injured person and the transport basket were again wrapped in plastic (Herculite) and turned over to ambulance personnel.
An ambulance stretcher and the injured person were also covered with two (2) sheets to further control any contamination. An HP Supervisor and technician then accompanied the ambulance to Charlotte Memorial Hospital.
At the hospital the injured person was examined by medical personnel, and then McGuire HP personnel surveyed and determined that he was free of contamination.
The hospital equipment used was also surveyed and determined to be free of contamination.
The Unusual Event was secured at 12:20 PM following a notification from HP personnel at the hospital that the injured person and the hospital were not contaminated.

It should be noted that Mr. A.L. Cunningham, Senior Radiation' Specialist from the NRC Region II Emergency Preparedness and Radiological Protection Branch was at i
McGuire at the time of the event performing an unannounced inspection, and went to i
Charlotte Memorial Hospital to observe the contamination control and medical treatment.
