Gentlemen:
This refers to the routine safety inspection conducted by Messrs. P. G. Brochman,
.P. L. Hiland, W. Grant, and S. P. Ray of this office on May 12 through July 7,
1989, of activities at Clinton Power Station authorized by NRC Operating License
No. NPF-62, and. to the discussion of our findings with Mr. J. S. Perry and
others of your staff at the conclusion of the inspection.
The enclosed copy of our inspection report identifies areas examined during
'the inspection. Within these areas, the inspection consisted of a selective
examination of procedures and representative records, observations, and
interviews with personnel.
During this inspection, certain of your activities appeared to be in violation
of NP.C requirements as specified in the enclosed Notice. A written response is
required.
We have reviewed the circumstances regarding the violations contained in this
report and even though these were Illinois Power identified items, we are
concerned with the observed management weaknesses and poor oversight control
which resulted in multiple examples of improper decisicns by Operating,
Engineering, and Quality Department personnel and inadequate control of
actions to satisfy Technical Specification requirements. These examples also
appur to illustrate a lack of attention to detail in operation of the Clinton
Plant. Please gSc these issues particular at%ntion in your responte.
In accordance blui 10 CfR 2,750 of the Commirston's regulations, a ccpy of
this letter, the enclosures, and your resprese to this letter will be pieced
in the NRC Public Document Room.
The responses directed by this letter and the accompanying Notice are not
subject to the riearance procedures of the Office of Management and Budget
as required by the Paperwork Reduction Act of 1980, PL 96-511.
