POLICY 1.1 It is the policy of Twin City Testing Corporation to develop and implement a program for shipping packages intended for the transport of radioactive i
sources.
1.2 Design and fabrication of shipping packages are not a part of this Quality l
Assurance Program. Certification of package approval from the manufacturer or supplier will serve as proof that packages meet NRC design and fabrication criteria.
i 1.3 This Quality Assurance Program fulfills the philosophy and aims of our Corporate Quality Assurance Program for maintaining a program of qualification and certification of personnel, equipment and materials used in our service to our clients.
2.
ORGANIZATION l
2.1 The organizational chart shows the relationship of company management and-l the personnel directly involved in this Quality Assurance Program for l
Shipping Packages.
i 2.2 The Radiation Safety Officer shall have overall responsibility for the program which includes training and certification of personnel, inspection, i
handling, storing and shipping of sources, document control and auditing i
the program.
2.3 Qualified personne1' designated by the Radiation Safety Officer will perform the various activities under the program.

QUALITY ASSURANCE PROGRAM i
3.1 The Quality Assurance Program established by Twin City Testing Corporation is designed to meet the requirements of 10 CFR 71 " Packaging and Transportation of Radioactive Material".
I 3.2 Work and Quality Assurance training procedures will be developed as I
necessary to cover each activity under this program.

procedures will be made by Department Manager and/or Radiation Safety Officer.
3.3 All documents necessary for this program will be accumulated and maintained by the Nondestructive Testing Department.
3.4 The Radiation Safety Officer will be responsible for the control and distribution of Manuals. work procedures and any other documents to authorized personnel who carry out any of the activities under this program.

- Revisions 3.5 The Radiation Safety Officer will be responsible for the indoctrination and training of individuals assigned to perform any of the activities under this procedure.
- 3. 6 This program will be reviewed at' least every two years or as necessary to keep it current with the latest requirements of the Nuclear Regulatory Commission or the Department of Transportation.
4.
DOCUtfENT CONTROL 4.1
' Procedures shall be developed for the control of issuing, distribution, use and revision of documents required under this program.
4.2 The Radiation Safety Officer will be responsible for the control of documents according to procedures established under this program and approved by the Department Manager, Nondestructive Testing.
5.
HANDLING, S1DRAGE AND SHIPPING 5.1 Activities performed under this program will be done according to safety procedures for the receipt, handling, storage and shipment of radioactive sources in exposure devices, source changers and the shielded shipping containers.
5.2 Shiments will not be made until all tests, certifications, acceptance and final inspections have been completed.
5.3 Radiography personnel will perform the functions described in this section.
6.
INSPECTION, TESTS AND OPERATING STATUS 6.1 The inspection, tests and operating status of shipping pachages for radioactive sources will be controlled by written work procedures.
6.2 The results of such inspections and tests and/or operating status will be recorded on forms designated for this purpose.
Such documents will be maintained in NDT Department files for reference or audit.
6.3 Non-conforming parts or packages will be noted on the reporting form and will be tagged " HOLD" to prevent unauthorized use.
6.4 Radiography personnel will perform these functions.
7.
QUALITY ASSURANCE RECORDS 7.1 Quality Assurance records documenting the activities under this program shall be kept in the NUT Department.
7.2 These Quality Assurance records shall include:

- Revisions 7.2.1 Quality Assurance Manuals 7.2.2 Work Procedures 7.2.3 Purchase Orders 7.2.4 Receipt Inspections 7.2.5 Maintenance Inspection Records 7.2.6 Inventory Records 7.2.7 Shipping Records 7.2.8.
Audit Records 7.2.9 Equipment Calibration Records 7.2.10 Nonconformance and corrective action reports where applicable 7.3 Records will be maintained in accordance with written procedures including identification, index, storage location and retention period.
7.4 Quality Assurance ' records shall be -dated and signed by authorized department personnel.
8.
AUDITS 8.1 Audits of the Quality. Assurance Program will be performed using written checklists.
8.2 Audits will be performed on the activities under this pro 6 ram at least once per year following an audit schedule.
8.3 Audit reports will, be. maintained by the NDT Department for review by authorized personnel.
