SUBJECT: NRC INTEGRATED INSPECTION REPORT NO. 50-327/98-04 AND

50-328/98-04 AND NOTICE OF VIOLATION

Dear Mr. Scalice:

This refers to the inspection conducted on March 15, 1998 through April 25,

1998, at the Sequoyah facility. The enclosed report presents the results of

this inspection.

During the inspection period, your conduct of activities at the Sequoyah

facility was generally characterized by good operations. However, negative

findings were identified for operator errors that led to a boration event and

for the failure of plant personnel to identify water intrusion into a vital

480 Vac motor control center. The conduct of maintenance and surveillance

activities was generally characterized as good, with a positive finding for

the observed condition of the Unit 1 ice condenser. However, a concern was

identified with the process for random sampling of ice baskets due to the

number of baskets which had become frozen-in-place and thus could not be

weighed. The conduct of engineering was good with a positive finding for

implementation of vendor guidance for monitoring and refurbishing reactor

protection system power supplies. Plant support was considered to be good

based on a special security inspection during this inspection period.

Based on the results of this inspection, the NRC has determined that a

violation of NRC requirements occurred. The violation is cited in the

enclosed Notice of Violation (NOV) and the circumstances surrounding it are

described in detail in the subject inspection report. The violation involved

a failure to perform the appropriate technical specification (TS) surveillance

requirement when the emergency diesel generator fuel oil transfer pump

automatic start circuitry was disabled.

i You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response. The NRC will \

use your response, in part, to determine whether further enforcement action is i

necessary to ensure compliance with regulatory requirements.
