discussed during the meeting (containment recirculation sump (sump) chronology
and issues, recent events, and management issues) are described in the enclosed
meeting report.
In the enforcement conference, we concluded that the conditions and events
related to the sump demonstrated a lack of effective management involvement
and accountability to assure safe plant operations and conditions. This
concern has been the root cause of many of Trojan's continuing events and
problems, and has hindered improvements at Trojan for some time. This serious
concern underlies continuing problems that have been previously discussed with
you:
ineffective communication of management expections, inadequate support
for the Quality Assurance organization, and ineffective implementation of the
,
root cause/ problem resolution program. We understand that you have taken and
plan to take actions to improve management performance.
' We also concluded that your understanding and implementation of design bases
for the sump was not adequate. Although PGE did take a lead in earlier years
in establishing an aggressive design basis document development program, the
lack of adequate design basis understanding and implementation has been a
continuing concern with PGE's performance in this area.
From the enforcement
conference, we understand that you plan to institute a re-review program for
DBD's including system walkdowns.
PGE management should establish objective, evaluative feedback systems to
ensure the actions proposed and taken achieve the desired results. We look
forward to you keeping us informed of your status on this important effort.
In accordance with 10 CFR 2.790 (a), a copy of this letter and its enclosure
will be placed in the NRC Public Document Room.
