Ens ssed for your records is NRC Form 591, " Safety Inspection." Please note that no violations were observed during the March 13, 1989, cobalt-60 teletherapy source change performed by your personnel at the University of Oklahoma, Department of Radiologic Training, Oklahoma City, Oklahoma. This activity was inspected by Messrs. W. L. Holley and G. M. Vasquez from our office. Your personnel performed this source change under reciprocity authorization from NRC. No response to this letter is necessary.
During the source change, the NRC inspectors observed Neutron Products procedures, techniques, surveys, and documentation. The NRC inspectors performed radiation and contamination wipe surveys of the shipping casks, source changer, vehicle, and portable tool chest / work bench.
