i SEQUOYAH NUCLEAR PLANT UNITS 1 AND 2 SPECIAL REPORT 89-05 DESCRIPTION OF EVENT On March 14, 1989, with unit 1 in mode 1 (100-percent power, 2235 psig 578 degrees F) and unit 2 in mode 5 (0 percent power, 260 psig, 110 degrees F),
a fire barrier (door A-158a, Auxiliary Building elevation 734, emergency gas treatment system (EGTS) filter housing room) was nonfunctional for an interval greater than seven days. The EGTS is common to both units and both trains of filter housing are located in the same room. The door is a 3-hour rated overhead rolling fire door. During performance of Surveillance Instruction (SI)-261, " Visual Inspection of ~ mbnical Specification Fire Doors on A Periodic Basis," the latch lever broke and rendered the fire door inoperable. A work request (WR) was issued to repair the door, and an hourly firewatch was established as required by Technical Specification (TS)
Limiting Condition for Operation (LCO) 3.7.12.
Subsequently, the door was rolled up and is being kept open by inserting a lock bolt through the gear mechanism. Maintenance Support determined that the replacement (latch lever) parts could not be procured in seven days due to the fact that the original manufacturer of the fire door is no longer in business. As a result, the fire door could not be repaired and returned to service in seven days and requires a special report in accordance with TS LCO 3.7.12.
j CAUSE OF CONDITION The surveillance test ensures that in the event of a fire, the overhead rolling fire door will operate as designed. This is done by disconnecting the fusible link (which melts during a fire) from the latch lever mechanism

and allows the door to close. While disconnecting the fusible link, the latch lever slipped after the door started closing and reengaged into the gears causing the latch lever to break.
The cause of the fire door remaining nonfunctional for greater than seven days is that the required parts for the overhead rolling fire door were nc; maintained in stock, and the original manufacturer of the fire door is no longer in business. As a result, procurement of the acceptable replacement parts has become time consuming.
ANALYSIS OF CONDITION This condition is being reported in accordance with requirements of action statement (a) of LCO 3.7.12.
I A roving firewatch was established to inspect the WTS filter housing room on an hourly basis as required by the action statement of LCO 3.7.12.
The existing fire detection and fire suppression systems for the room are operable and would activate in the event of a fire.
In addition, the door l
opening has a heavy equipment door similar to other heavy equipment doors i
that have been evaluated as equal to a 3-hour fire door.
This heavy l
equipment door is controlled by the plant security key card system.
l Therefore, there is no danger to safety-related equipment.

CORRECTIVE ACTION A roving firewatch at a regular interval of one hour was established and will be maintained until the fire door is repaired and returned to functional status. A vendor, who possibly could supply the original latch lever, has been located but the parts delivery has not been confirmed. A procurement request to purchase the required parts has been initiated, and efforts are presently being pursued to procure the parts from available sources. WR B-282544 will be used to document the door repair.

anticipated that the door will be repaired and returned to functional status by June 15, 1989.
