Dear Mr. Lanza:

This letter addresses the inspection of your facility at Hillside, New Jersey conducted by Messrs. R. L. Cilimberg, R. P. Correia, and W. Shier, of this office on November 2-4,'1988 and February 1-3, 1989, and the discussions of their findings.with Messrs. Dean Lanza and Hamilton Vazquez of your staff at the conclusion.of the inspection.

The purpose of the inspection was to follow-up an allegation that nonconforming material was supplied to the Anchor Darling Valve Company (ADV)

- by the Meredith Corporation.

Areas examined during the NRC inspection and our findings are discussed in the enclosed report. This inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by staff inspectors. Release of this report was delayed during NRC's ongeing review of nonconforming and substandard vendor products.

During-this inspection it was found that the implementation of your QA program failed to meet certain NRC requirements. Heredith certified and supplied stock material as meeting the requirements of Section III of the ASME Boiler and Pressure Vessel Code to the ADV, South Texas nuclear plant and to Congdon and Carpenter without testing each piece of material. Meredith had not audited

~ the quality system programs at American Tank and Fabricating Company or H. T.

Potts to establish that they had quality systems programs that met the require-ment of NCA-3800. In addition, nuclear sales order folders were not color coded in accordance with Neredith's procedures. The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.

Please provide us within 30 days from the date of this letter a written statement containing: (1) a description of steps that have been or will be taken to correct these items identified in the enclosures; (2) a descri of steps that have been or will be taken to prevent recurrence; andthe(3) ption dates your corrective actions and preventive measures were or will be completed.

We will consider extending the response time if you can show good cause for us  ;

to do so.
