NRC INSPECTION REPORT N05. 50-338/88-36 AND 50-339/88-36
This ~ refers to the Nuclear Regulatory Commission (NRC) inspection conducted
' by J.- L. Caldwell on December 31, 1988 - February 2, 1989.
The' inspection
included a review of activities authorized for your North Anna facility.
At.
the conclusion.. of the inspection, the findings were discussed'with those
- aembers of your staff identified in the enclosed Inspection Report.
Ar'eas .' examined during the inspection are identified in the ' report.
Within;
theseLareas, the inspection consisted of selective examinations of procedures
and representative ' records, interviews with personnel, and observation of '
activities in progress.
The inspection findings indicate that certain activities appeared to violate
NRC requirements.
The apparent violation described in the enclosed Inspection
Report is'under consideration for escalated enforcement action.
Accordingly,
no Notice of. Violation is being issued at this time, and no response to this
letter is required.
However, please be advised that the nuuber and
characterization of the violation described in the enclosed Inspection-Report
may change as a. result of further NRC review.
An Enforcement Conference was
held on February 7.1989 to discuss the apparent violation. A summary of Ois
Enforcement Conference was sent to you on March 2,1989.
You will be advised
by separate correspondence of the results of our deliberations on this matter.
Your attention is ir.vited to the unresolved item identified in the Inspection
Report. This matter will be pursued during future inspections.
Also, seven additional examples of a pre <iously identified violation,
338,339/88-31-02, were identified during the inspection' and are included in
this-report.
In that your response of January 23, 1989, was comprehensive and
we believe that these additional. examples are encompassed by your ongoing
programs, no further response is required.
We expect that your efforts will
rapidly resolve this significant personnel error trend. We will be monitoring
your actions on this matter.
